Richland County restaurant inspections: critical violations recorded April 18-24

These restaurant inspections with critical violations were conducted by Richland Public Health between April 18 and April 24:

In-N-Out Mart #103, 300 N. Mulberry St., Mansfield, April 18. Handwashing sink not accessible (critical, corrected during inspection). Hand wash sink had milk in sink well. Employee removed milk and discarded container. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed no quat sanitizer in three-compartment sink mixer. Manager adjusted mixer and quat was at sufficient amount. TCS foods not being cold held at the proper temperature (critical, corrected). Observed milk in walk-in cooler being stored at 53 degrees. Employee discarded milk during inspection.

Ashland Road Duchess #1103, 1000 Ashland Road, Mansfield, April 18. Handwashing sink not accessible (critical, corrected during inspection). Observed coffee urn positioned over hand sink station. Keep area clear to promote use. Manager moved urn during inspection.

Ontario Estates Senior Living, 2010 Walker Lake Road, Mansfield, April 19.Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Can opener blade had visible residue. Person in charge (PIC) ran can opener through warewashing machine. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Food storage containers in clean area had visible residue on food contact surfaces. PIC removed to warewashing area. Presence of live insects (critical). Small flying insects observed in kitchen. Please have copy of most recent pest control invoice and remove by re-inspection.

Wokano, 2284 W. Fourth St., Ontario, April 22. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed a straw in the hand sink. Discussed hand sinks are to be used for hand washing purposes only. PIC removed straw at the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected). Observed an uncovered container of fish in the walk-in cooler. PIC placed a lid on the fish.

Ontario High School, 467 Shelby Ontario Road, Mansfield, April 22. Unapproved chemical sanitizer used on food contact surfaces (critical, corrected during inspection). PIC states observed spray bottle of Spic and Span sanitizer is used on prep tables and surfaces in kitchen. Discussed ensuring sanitizer used on prep surfaces and food contact surfaces is approved for use on food contact surfaces. Discontinue use of Spic and Span on prep and food contact surfaces. At re-inspection, discussed that Spic and Span is not to be used on food contact surfaces and instead an approved food grade sanitizer should be used. PIC verbalized understanding.

Arby's #7585, 689 Lexington Springmill Road, Mansfield, April 22. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). Observed cooked mozzarella sticks and chicken tenders held below 135 degrees on hot holding unit next to fryer. PIC voluntarily discarded during inspection. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed can of whipped topping stored in a metal pan on ice to have an internal temperature of 46 degrees. PIC verified can was just placed there at opening and placed in walk-in freezer to rapidly cool back down to 41 degrees.

Gionino's Pizzeria, 1289 Ashland Road, Mansfield, April 24. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener blade with ketchup residue. Clean can-opener blade between each use. Manager cleaned blade during inspection.
