Springfield restaurant put on risk control plan after inspection finds live cockroaches

After live cockroaches were found in hand-washing sink and box of food, a Springfield restaurant is on risk control plans for "pest control and pest monitoring," according to a March 25 Springfield-Greene County Health Department food inspection report.

The inspector documented live cockroaches in a sink and in a box of dry miso soup mix, as well as "many dead insect carcasses" in the kitchen at Misaki, 1635 N. Glenstone Ave. Compliance checks for the risk control plans will start the week of April 1, according to the report.

The restaurant was also cited for not using its three-vat sink for sanitizing, employees washing their hands in the three-vat sink and not having any sanitizing solution on site. The lack of sanitizer was solved when the manager had an employee purchase sanitizer, according to the report.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

One priority violation: Observed sausage at 46 degrees Fahrenheit, beef at 46 degrees, pasta at 46 degrees, tomatoes at 45 degrees, chicken at 46 degrees and ham at 43 degrees in pizza prep cooler, which was corrected by voluntarily discarding food or moving it to the walk-in cooler. Re-inspection will occur. No non-priority violations.

Oasis Conference Center, 2546 N. Glenstone Ave., Springfield. March 21 routine inspection – Result: permitted with re-inspection. A re-inspection will occur March 25 to re-check women's restroom hand sink for running water. One priority violation: Observed two metal spoons stored dirty with food debris, one slicer stored dirty with food debris, which was corrected by washing, rinsing and sanitizing utensils. Four non-priority violations: Observed highest hand-washing sink temperature is 73 degrees to 78 degrees; observed hand sink in women's restroom does not have running water; observed buildup of dust and debris in air vents throughout facility, buildup of grease and food debris on walls behind kitchen equipment, buildup of grease and food debris on floors under and behind kitchen equipment; observed single service cups, napkins and plates were uncovered without wrapping and opened from top seam in storage area, which was corrected by discarding items.

Four non-priority violations: Observed pizza slice sitting at 88 degrees on pizza rack without time control records, which was corrected by providing education on time control records and discarding pizza; observed no sanitizer test kit on site, which was corrected by West concessions providing test strips; observed mouse feces on floor of cabinets under soda dispenser; observed dead mouse on floor of cabinet under soda dispenser, which was corrected on site by sanitarily removing mouse carcass.

Ozarks Empire Fairgrounds - E Plex Concessions West, 3001 N. Grant Ave., Springfield. March 22 routine inspection – Result: active. No priority violations. Two non-priority violations: Observed plumbing leak in cabinet under soda dispenser, which is being contained with plastic tote and will need to be repaired or replaced; observed three boxes of pre-cooked hamburger patties thawing in two-door stainless steel fridge with ambient temperature of 52 degrees. Boxes were at 27 degrees, and were moved to two-door glass fridge, which had ambient temperature of 40. Only non-potentially hazardous foods will be stored in two-door stainless steel fridge until it is repaired or replaced.

One priority violation: Observed three dairy smoothie mixes sitting in wells without refrigeration or ice baths, ranging between 53 degrees to 59 degrees, which was corrected on site by discarding mixes and will have time control methods implemented going forward. One non-priority violation: Observed three dairy smoothie mixes sitting in wells without refrigeration or time control procedures, which was corrected by discarding mixes and creating written procedures and records during inspection.

Panda Express, 720 N. Glenstone Ave., Springfield. March 21 routine inspection – Result: active. No priority violations. One non-priority violation: Observed water at 56 degrees in men's bathroom hand-washing sink.

Pappo's Pizzeria & Pub, 900 E. Battlefield Road, Springfield. March 21 routine inspection – Result: active. Two priority violations: Observed chicken wings and meatballs at 115 degrees to 132 degrees in hot hold unit, which was corrected by reheating items to over 165 degrees and turning unit up; observed vacuum packaged cooked refrigerated product dated March 15 including pork sausage, chicken wings and shredded pork in reach-in cooler. Items were cooked, cooled and vacuumed packed outside of the county and an approved HACCP plan must be submitted.
