  • DUNKIN DONUTS (771 PRINCE AVE): 89. Inspection notes: "Mold growth observed in cavity of ice maker at front area. Coffee filters stored without protection on top of coffee brewer. Missing floor drain grate in men's restroom."

  • MELLOW MUSHROOM (320 E CLAYTON ST): 87. Inspection notes: "Menu reminder and disclosure for undercooked foods not correct. Employees wearing bracelets during food preparation. Food pans stacked wet after washing. Dish machine racks stored on floor in wash room. Food contact surfaces of wrapped silverware exposed during storage. Dust and food debris accumulation on fan guards in walk-in (WI)."

  • BARBERITOS (680 N 29 HWY STE 250): 88. Inspection notes: "Sanitation agent not the same in wash and sanitize step for cleaning dining room tables (Dawn soap used for wash step and Quat used for sanitation step). Thermometer not sanitized in between uses at prep table - thermometer was only washed off. No handwashing sign in women's restroom used by employees notifying employees to wash hands. Bowl used as scoop in bulk sugar container with no handle. Ice scoop stored on top if ice machine not protected. Observed stacks of clear bins stacked wet on clean dish shelf over prep table. Multiple clear bins have cracked/chipped lids throughout establishment. All air vents in back kitchen have heavy dust buildup. Missing ceiling tile over door of walk-in cooler. Wet mop stored in mop bucket - not hung to dry."

  • ST. MARY'S COFFEE SHOP (1230 BAXTER ST): 89. Inspection notes: "Storage of brushes in hand wash sink. Food probe thermometer not sanitized before each use. Employee cell phone storage on work surface."

  • MAEPOLE (1021 N CHASE ST): 89. Inspection notes: "Gasket fraying on right hot cabinet. Prep cooler not keeping TCS foods at 41F or less."

Observed accumulations of debris on/in light shield of WIcooler. Observed accumulations of grease, oils, flour and dust on ceiling vents of kitchen, dish and storage areas."
