'Serious concerns' raised after prisoner dies

A coroner had "serious concerns" after she heard evidence about the death of a prisoner.

Mohammed Azizi, 32, died on 15 May at the Norfolk and Norwich University Hospital. The inquest jury heard he had Crohn’s Disease, refused food and died from cardiac atrophy, malnutrition and pulmonary embolism.

Coroner Samantha Goward said she was concerned after prison officers claimed under oath that signatures on documents were not theirs.

The BBC has asked the Ministry of Justice for comment.

Ms Goward, an area coroner for Norfolk, said she was not satisfied future deaths would be prevented.

She cited claims that documents had been altered, after being signed, and that documents were retrospectively completed.

She said that two prison officers, who gave evidence, said signatures of their names had not been written by them.

Ms Goward also said she had requested evidence from HM Prison Service, but that it had not been submitted to the court in time to be admitted.

She said: "There is a risk a full picture is not being received and could lead to the same thing happening.

"I will therefore release my report that there is a concern of future deaths if documents are not properly kept and if there is not full disclosure."

Mr Azizi, from Waltham Abbey, Essex, was sentenced to more than five years in prison for arson in 2021.

At his trial, Chelmsford Crown Court heard he had started a number of fires, destroying two garages and nine vehicles.

But he complained food in prison made him unwell owing to his Crohn's diagnosis and that he had not been given the food he had requested.

The jury returned a narrative conclusion, setting out his medical causes of death.

Ms Goward said she would submit her "Prevention of Future Deaths" report to the governor of HMP Norwich, HM Inspectorate of Prisons and HM Inspectorate of Probation.
