  • The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions related to food employee certification and food safety.

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • Observed unauthorized persons in in food preparation back room and food customer service areas.

  • The Person in Charge failed to control access to food facility by unauthorized persons.

  • Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between.

  • Food employee observed donning single-use gloves without a prior hand wash.

  • Food employe observed in customer service/food preparation area, not wearing proper hair restraints, such as nets, hats, or beard covers.

  • Facility does not have written procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility.

  • Food dispensing utensil in numerous foods in the sandwich preparation unit observed stored in the food and not with handle above the top of the food and container.

  • Observed in-use ice scoop stored on the top of the ice machine in direct contact with surfaces that are not clean to sight and touch.

  • Observed wiping cloths in extremely unclean condition in rear and front food preparation areas as well as the three-basin sink area.

  • Observed wet wiping cloths in rear and front food preparation areas not being stored in sanitizer solution.

  • Cooked chicken, refrigerated ready-to-eat time/temperature control for safety foods, inside the walk-in cooler, were not compliant with datemarking by being labeled with a discard or use-by date of not more than 7 days, and requires discarding.

  • The quaternary ammonia concentration in the sanitizing solution of the 3-bay warehouse sink was 0 ppm, rather than 150-400 ppm as stated on the manufacturers use directions.

  • Observed black moldlike substance on interior of ice chest.

  • Non-food contact surfaces (cold unit gaskets) not cleaned at a frequency to preclude accumulation of dirt and soil.

  • Wet wiping cloths are heavily soiled and stained.

  • Observed fountain soda machine nozzles in liquid filled bucket on the floor under the three basin sink.

  • Observed single use cups and lids stored directly under the hand sink wastewater pipe.

  • Observed clean food utensils stored on the spigot/faucet apparatus of the three basin sink.

  • Observed single-service, single-use articles stored directly under hand soap dispenser where exposed to splash and chemicals.

  • The food facility does not maintain Food Employee Certification records, as required.

  • Food facility does not employ a certified food employee, as required.

  • The handwash sink in food preparation area does not have single-use towels, continuous towels, or air drying device.

  • No sign or poster posted at the handwash sink in the food preparation area to remind food employees to wash their hands their hands.

  • First aid supplies being stored in rear preparation area, in direct contact with single-use gloves, and a possible source of contamination.

  • Observed chemicals stored in rear preparation area on shelving with and above single use service items, utensils, equipment.

Violations (Inspector Comments):

  • Ther Person in Charge did not demonstrate adquate knowledge of food safety in this food facility, as evidenced by this non-compliant inspection.

  • Refrigerated ready-to-eat, time temperature control for safety food including, but not limited to, sweet potatoes in sauce, mashed potatoes, cooked meats, cooked vegetables etc. prepared in the food facility and held for more than 24 hours, located inside the walk-in cooler, are not being date marked.

  • Paper towel dispenser empty at the handwash sink in the food preparation area.
