Winter Haven Hospital announced that some patient information had been accidentally disclosed.

The incident occurred March 15, when an employee, while emailing forms to a patient, inadvertently attached a cardiac rehabilitation department file containing information about other patients, corporate owner BayCare said in a news release.

The employee identified the error and contacted the recipient, who stated that the file would be deleted, the release said. The information disclosed included cardiac rehabilitation patient names, dates of birth, the procedure requiring cardiac rehab, date of service and, in some cases, email addresses and phone numbers, BayCare said.

It is not believed that any of the information will be used in an inappropriate manner, the release said, and Winter Haven Hospital has taken corrective actions to help prevent a re-occurrence. The hospital began mailing letters to patients whose information was involved in the incident on May 8. Anyone who believes their information was involved and has questions may contact BayCare at 855-466-6677, Monday through Friday from 8 a.m. to 4:30 p.m.
