Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 1 and May 7:

Pronto Pizza, 914 Ashland Road, Mansfield, May 1. Working containers of poisonous or toxic materials not properly labeled (critical). Bottles of clear blue liquid were not identified. Correct by May 3.

Bellville Elementary School, 195 School St., Bellville, May 1. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed a container and squeeze bottles of ranch made in the facility at 46 degrees in the cooler with the bottled drinks. Person in charge (PIC) voluntarily discarded ranch dressing.

Mickey Mart #21, 750 Ohio 97 W., Bellville, May 1. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated Bunn coffee brewer cleaning solution tablets are used to clean the coffee pot. Food contact surfaces must be washed, rinsed, sanitized and air dried. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected). PIC unable to locate written health policies. Provided RPH's employee illness reporting agreement form. Improper storage of poisonous or toxic materials (critical, corrected). Observed weed killer on a shelf above the three-compartment sink.

Denny's #9605, 720 N. Lexington Springmill Road, Mansfield, May 2. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected during inspection). Observed multiple working buckets of sanitizer solution to be at 0 ppm. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed containers of cut strawberries and cut spinach in reach-in cooler not date marked. Presence of live pests (critical). Observed small flying insects in servers' area, near dry goods storage and in utility area. Rid observed pests by May 10.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, May 2. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical repeat). PIC states cleaning (wash, rinse, sanitize) frequency of outer surfaces of steam wand on espresso machine in Starbucks area is at the end of every day. Discussed that the outer surfaces of the steam wand is to be fully cleaned at a minimum of every 4 hours that it is in use. Observed no change at re-inspection. Retrain/re-educate employee and implement appropriate cleaning frequency by re-inspection May 9. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by May 9. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed milk frothier/steam wand in the Starbucks area to not be cleaned at the required four-hour cleaning frequency. Presence of live insects (critical repeat). Observed multiple live insects heavily concentrated in the deli back area. Re-contact a licensed pest control operator and thoroughly clean area by re-inspection May 9.

The Waterford At Mansfield, 1296 S. Trimble Road, Mansfield, May 2. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Sanitizer solution was at 0 ppm. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected). Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Jell-O in walk-in cooler left uncovered. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical, corrected). Sanitizer solution at 0 ppm. TCS foods not being cold held at the proper temperature (critical, corrected). Hot dogs in make table had internal temperature of between 42-45 degrees for an undetermined amount of time. PIC voluntarily discarded 3 pounds. Food contact surfaces not easily cleanable (critical, corrected). Cracked food contact storage container observed in cleaning area. PIC removed container from service. Food contact surfaces not easily cleanable (critical, corrected). Scissors with visible food residue could not be separated for cleaning. PIC removed from service. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Wash temperature was 93 degrees in three-compartment sink.

Joe & Mary's Market, Adm. Sturges Inc., 162 Sturges Ave., Mansfield, May 2. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC could not state the correct cleaning procedures, even stating that the Icee machine parts are cleaned in the handwashing sink. Ensure that all equipment is cleaned in the three-compartment sink and the handwashing sink is only used for handwashing. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed raw ground sausage stored over bologna in the retail dairy cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed bag of sugar at the front coffee station that was not covered. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Observed motor oil, ammonia and car wax stored on top of the ice chest in the back stock room. Presence of live rodents (critical repeat). Observed rodent droppings under the mop sink and along the back wall near mop sink of the back storage area. Facility is to completely remove all evidence of rodent droppings, contact a licensed pest control operator, clean and completely sanitize area by re-inspection May 9.

Panda Express, 994 N. Lexington-Springmill Road, Ontario, May 3. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed containers of salt, soup base and oils sitting uncovered/unprotected at rice cook station and on top of cooked rice holding cabinet. TCS foods not being cold held at the proper temperature (critical, corrected). Observed garlic in oil as well as garlic/ginger mix held in top portion of prep cooler along cook line to have internal temperatures above 41 degrees. PIC placed on ice bath to rapidly cool to 41 degrees or below.

Ontario Stingel Elementary, 416 Shelby Ontario Road, Mansfield, May 3. No air gap on the plumbing system (critical, corrected during inspection). Observed air break at drain from prep sink but no visible air gap.

Crestview High School, 1575 Ohio 96 E., Ashland, May 3. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer measured only 80 ppm.

Fair Board Auxiliary - Fairhaven Hall, 750 N. Home Road, Mansfield, May 4. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed bologna and ham in the top of the make table cooler with an internal temperature of 50 degrees. PIC voluntarily discarded ham and bologna.

Fair Board Auxiliary - Youth Hall Cafeteria, 750 N. Home Road, Mansfield, May 4. Improper bare-hand contact with RTE foods (critical, corrected during inspection). Observed a volunteer touch ready-to-eat food without gloves.

Buckeye Community School, 1404 Park Avenue West, Mansfield, May 6. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Multiple critical violations observed involving cold hold and hot holding foods. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected). Proper instruction was given regarding the holding of foods below 41 degrees and above 135 degrees. TCS foods not being hot held at the proper temperature (critical, corrected). Cheese broccoli soup was observed hot holding at 115 degrees on food line. PIC voluntarily discarded soup. TCS foods not being cold held at the proper temperature (critical, corrected). Tossed salad was in container on food line without any ice or cold holding and measured at 60 degrees. PIC voluntarily discarded salad. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Two containers of marinara sauce date marked to be discarded by May 4 were observed in two-door cold holding unit. Listed items were voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Lunch meat in two-door cooler was observed date marked May 6. Listed item was voluntarily discarded.

Spanish Immersion, 240 Euclid Ave., Mansfield, May 6. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Thermometer probe was utilized without first sanitizing food contact surface before and after.

Knights of Columbus, 250 Vernon Road, Shelby, May 6. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed degreaser and detergent being stored with clean equipment. Presence of live rodents (critical). Observed rodent dropping in both of the clean equipment storage areas and within the cabinets within the same room. Completely clean and disinfect the areas and contact pest control operator before re-inspection May 13.

Kimmyscucina, 275 E. Main St., Lexington, May 7. Presence of live insects (critical). Observed gnats on the wall behind the Argus two-door freezer in the back room. Correct by May 14.
