Observed poisonous or toxic materials stored in a manner that has the potential to contaminate food. At the time of inspection observed three live cockroaches in the bar area, two live cockroaches on wall and near three compartment sink, one live cockroach near cash register area. Utensils during a pause in use stored incorrectly. Gasket in reach in cooler in bar area was damaged. Trash and food debris build up on the floors of the bar area. Faucet for three compartment sink leaking.

Result: Immediate closure

Person in charge not knowledgeable in food safety due to multiple prior violations. Instructed person in charge to increase knowledge in food safety by attending manager certification course. Employee medication improperly stored. Spray paint and sanitizer spray bottle stored on prep table left of ice machine. Chicken improperly thawing. Food temperature measuring devices were not provided or readily accessible. Food that is not readily recognized removed from original packaging without proper labeling. Wiping cloths improperly used. Employee drinking in an undesignated area. Food employee didn't wash hands as required. Hand washing sink blocked or inaccessible.

Pastians Bakery, 3320 Second St. NW

Facility had five priority violations and seven priority foundation violations. Facility had evidence of ongoing rodent infestation. Person in charge failed to ensure employees follow necessary procedures. Food packaging not in good condition. Various articles were stored inside open bags of flour. Equipment had build up of soil residues. Facility did not have sanitizer made upon arrival. Rolling garage in production area in very poor repair with gaps present. Rodent droppings throughout the bakery processing area, distribution area, particularly inside bakery equipment, dry storage area, mechanical room, on speed racks. dough portioning and scraping machine. Instructed person in charge to remove any dead pests. Snap trap was inside machinery with open dough inside the machinery which is a rodent was caught in the snap trap is could have propelled mouse particulates such as hair, blood or other matter onto the surfaces of food. Employee used soiled cloth glove. Hot water took multiple minutes to arrive. Plumbing system not maintained or in good repair. Rodent infestation throughout entire facility. Facility has been downgraded three times, on Feb. 26, April 1 and May 17. Food processing operations are to be immediately discontinued.
