Marion County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Marion Public Health between March 19 and March 26:

Pleasant Middle School/High School, 1117 W. Owens Road, Marion, March 19. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed prewashed apples not wrapped or covered for student self-service. Person in charge (PIC) had food employees wrap the apples.

DB's Rock'n Wing Truck, 5425 Stauffer Road, Morral, March 19. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed a cutting board that was heavily scored during the inspection. Operator flipped the cutting board and will use the clean side until new one arrives.

Pleasant Elementary School, 1105 Owens Road W., Marion, March 19. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed the mechanical dishwasher with chlorine levels above the approved concentration. Food employees will use the composite sink with quat sanitizer to rinse and sanitize the dishes after they have been washed in the dishwasher until it's repaired.

Subway #48645, 691 W. Center St., Marion, March 20. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed no air gap for the soda machine and ice machine drain lines.

Center Street MotoMart #4129, 691 W. Center St., Marion, March 20. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed a build-up of debris on the cheese and chili dispensers. PIC cleaned the dispensers.

Burger King Restaurant #1057, 1278 Mt. Vernon Ave., Marion, March 21. Improper bare-hand contact with RTE foods (critical, corrected during inspection). Observed PIC's bare hand touching the side of a French fry when putting the French fries in a to-go bag. PIC was retrained on food handling requirements and instructed to wear gloves or use a napkin to prevent bare-hand contact. Food contact surfaces not easily cleanable (critical, corrected). Observed a cracked/broken food container and cracked/broken container lids on the dish shelf. PIC discarded the damaged items.

Rally's #4054, 412 N. Main St., Marion, March 21. Handwashing sink not accessible (critical, corrected during inspection). Observed an empty food container in the hand sink. TCS (time/temperature control for safety) foods not being cold held at the proper temperature (critical, corrected). Observed cheesecake filling in the prep cooler with a temperature above 41 degrees. PIC discarded the cheesecake. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed a container of whipped topping in the prep cooler without a date mark. PIC discarded the whipped topping.

Adkins Dairy Queen Inc., 561 N. Main St., Marion, March 21. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed an open container of smoothie mix that had a temperature above 41 degrees in the top part of the prep cooler. PIC discarded the smoothie mix. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected).

Observed everything sauce made in-house that was stored in the walk-in cooler date marked for 10 days instead of seven. PIC fixed the date labels. Improper use of time as a public health control - 4 hours (critical, corrected). Observed everything sauce and re-hydrated onions not time stamped for time-in-lieu of temperature. PIC time stamped the items.

Kentucky Fried Chicken, 1917 Marion-Mt. Gilead Road, Marion, March 26. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed dishes that were still dirty stored in the dish drying area above the three-bay sink. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Observed some of the chicken on a tray in one of the warmers with internal temperatures below 135 degrees. PIC removed the chicken to be discarded. No air gap or approved backflow prevention device on the plumbing system (critical). Did not observe a backflow preventer on the pressurized hose connected to the mop sink. Correct by April 2.

Marion Pointe, 409 Bellefontaine Ave., Marion, March 26. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed a container with cooked mixed veggies past its seven-day hold time in one of the coolers. Food employee discarded the food.

DeWolfe Place, 1140 Wilson Ave., Marion, March 26. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed dried food on bowls after they had been cleaned and stacked to be put away. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed open bags of spinach and cut lettuce without date marks. PIC discarded the food items. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed TCS food items past their seven-day hold time in the coolers. PIC discarded the food.

Bob Evans Restaurants LLC #130, 188 American Blvd, Marion, March 26. Improper bare-hand contact with ready-to-eat foods (critical, corrected during inspection). Observed a server spill a plate of country fried steak onto another plate in the service window and pick up the steak with bare hands, placing the steak on the original plate to be served. Server was corrected regarding bare-hand contact and had the steak remade; the improperly handled steak was not served. Handwashing sink is being used for purposes other than handwashing (critical, corrected). Observed hand sinks in the kitchen being used for purposes other than handwashing. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected).

Observed food containers not covered in the grill line coolers as well as a container of sugar on top of the fryer exhaust hood that was not covered. Observed raw ground beef and pork sausage being stored over raw egg products in the walk-in cooler. PIC rearranged the storage in the walk-in cooler in order of minimum cooking temperatures. Improper use of time as a public health control - 4 hours (critical, corrected).

Observed hot cake gun with batter in it not labeled with a four-hour time stamp on the grill line and the batter temperature above 41 degrees. PIC placed a time stamp on the batter. Food contact surfaces not easily cleanable (critical, corrected). Observed damaged/cracked food containers and knives with broken blade tips. PIC discarded the containers and knives.

Comfort Inn, 256 Jamesway, Marion, March 26. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed waffle mix above 41 degrees in the waffle dispenser. PIC will use time as a public health factor and will discard within four hours of being placed in the waffle dispenser.

Marion General Hospital - Two South, 1000 McKinley Blvd., Marion, March 26. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected during inspection). Observed ice cream in freezer that looked melted and refrozen. PIC threw away ice cream.
