Food inspectors find live cockroaches crawling around Springfield restaurant

Live cockroaches troubled a Springfield restaurant this week, according to Springfield-Greene County Health Department food inspection reports.

A cockroach crawled across a counter during an inspection at Misaki 2, 2710 S. Campbell Ave., according to an April 29 report. The cutting board it touched and a knife were removed to be cleaned and any open food items on the counter were voluntarily discarded. Additionally, cockroaches were found around outlets, cooler motors and glue traps at Misaki 2, 2710 S. Campbell Ave., the report said. The report instructed the restaurant to "constantly be monitoring for cockroaches and find ways to minimize their presence." The restaurant needs a re-inspection.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

417 Taphouse, 431 S. Jefferson Ave., Springfield. April 26 routine inspection – Result: active. No priority violations. Two non-priority violations: Observed seals around make table one doors were falling off door interiors; observed ceiling in back room with cooler and freezers has active leak.

Ariake, 1110 E. Battlefield Road, Springfield. April 25 routine inspection – Result: permitted with re-inspection. Re-inspection scheduled for April 30 to re-check cold holding temperature of sushi prep top cold holder No. 1. One priority violation: Observed shrimp between 48 degrees Fahrenheit and 52 degrees in walk-in cooler, which was corrected by discarding shrimp. Rest of walk-in cooler was within acceptable temperature range. However, in sushi prep top cold holder No. 1, the salmon was at 48 degrees, squid at 52 degrees and tuna at 46 degrees, all of which was discarded. A re-inspection will take place April 30 to re-check temperature of sushi prep top cold holder No. 1. One non-priority violation: Observed walk-in cooler shelves have built-up dust and food debris, which was corrected on site with cleaning.

Ariake, 1110 E. Battlefield Road, Springfield. April 30 re-inspection – Result: active. Re-inspection conducted to re-check sushi prep top cold holder No. 1, which is now running in acceptable temperature range. No violations noted.

Arris' Pizza - Republic, 738 E. U.S. Highway 60 East, Republic. April 26 re-inspection – Result: active. Priority items corrected. Foods held at proper temperature in prep cooler and slicer cleaned. No violations noted.

Beef-A-Roo, 1411 W. Kearney St., Springfield. April 25 routine inspection – Result: active. No priority violations. Four non-priority violations: Observed hot water shut off at hand sink due to leaking faucet; observed staff unable to find any quaternary ammonia test strips for checking sanitizer; observed caulking and wall behind three-vat sink was molding; observed drain under three-vat sink is leaking.

Big Momma's Drive-Thru/Queen City Soul Kitchen, 1626 N. Booneville Ave., Springfield. April 26 routine inspection – Result: permitted with re-inspection. One priority violation: Observed cheese at 44 degrees in reach-in refrigerator. One non-priority violation: Observed boxes of single-serve items stored on floor, which was corrected at time of inspection.

Cabos Bonitos Grill & Cantina LLC, 406 S. South Ave., Springfield. April 30 routine inspection – Result: permitted with re-inspection. One priority violation: Observed no sanitizer level detected when dish machine was operated. Owner changed bucket and primed line but still did not get any residual; company contacted for service. Three-vat sink will be used to sanitize dishes until machine is fixed. Three non-priority violations: Observed chicken cooling in walk-in cooler while covered with plastic wrap, which was corrected by pulling back corner of wrap to allow heat to vent; observed walk-in cooler No. 2, reach-in cooler, make table, make table No. 2 and make table No. 3 do not have thermometers inside to measure and monitor ambient air readings; observed wall covering located by spray hose and dish machine was coming away from the sheet rock.

Two non-priority violations: Observed some food items prepared and packaged on-site for self-service that lacked ingredient labels; observed bag of cooked rice thawing in sink at room temperature, which was discarded by staff.

Drury University - Einstein Bro. Bagels, 900 N. Benton Ave., Springfield. April 26 routine inspection – Result: active. No violations noted.

One priority violation: Observed wedger attached to wall has buildup of dried food debris, person in charge unsure of when previously used, which was corrected on site when wedger was dismantled and parts run through dishwasher. Two non-priority violations: Observed lack of sanitizer in buckets that cloths are stored in, which was corrected on site by changing out and re-checking buckets; observed ceiling tiles in dish room need replaced.

Hardee's, 3465 W. Chestnut Expressway, Springfield. May 1 routine inspection – Result: active. Hot water was 84 degrees when inspector arrived and hot water heater was out, but staff was able to get pilot light relit and water heater running before inspector left. One fire extinguisher near back door needs replaced. One priority violation: Observed hot water was at 84 degrees and pilot light was out on inspector's arrival, which staff were able to correct by relighting the pilot light and warming water before inspector leaving. Four non-priority violations: Observed two containers of cooked sausage with lids being cooled in walk-in refrigerator; observed three wet mops on floor or in mop bucket; observed some grouting and coving in poor repair; observed floor around cooking oils storage tank is very greasy.

Hong Yen, 2922 S. Campbell Ave., Springfield. April 25 routine inspection – Result: active. Six priority violations: 1. Observed foods being held at room temperature with not marking for time as control, including boiled chicken at 83 degrees, beef at 108 degrees, noodles at 111 degrees and pho at 76 degrees. Manager stated they use items until they sell out; discussed that foods cannot be left out at room temperature without being marked for time as control. Manager said they would cold hold from this point forward. Corrected on site. 2. Observed no consumer advisory on menu in facility that offers rare beef pho and undercooked eggs. 3. Observed employee placing ingredients for spring rolls into spring roll wrappers and rolling spring rolls with bare hands. Items touched with bare hands were discarded and gloves were donned before returning to work. Corrected on site. 4. Observed knives stored on magnetic rack had food debris on them, which were removed from service and taken to dish machine. Education provided and corrected on site. 5. Observed potentially hazardous foods cooked and cooled the day prior not being at or below 41 degrees, such as soup at 47 degrees and noodles at 47 degrees, which were voluntarily discarded to correct violation. Education provided. 6. Observed employee medicines stored in multiple locations in kitchen area, which was corrected on site and education provided. One non-priority violation: Observed ice machine running into clogged mop sink, which is full of water. Manager said he would unclog drain.

HuHot Mongolian Grill, 2825 S. Glenstone Ave., Springfield. April 24 routine inspection – Result: permitted with re-inspection. One priority violation: Observed salad not being held at or below 41 degrees in salad cooler, which was voluntarily discarded. No non-priority violations.

Jimmy John's No. 817, 1002 S. Glenstone Ave., Springfield. April 29 routine inspection – Result: permitted with re-inspection. Two priority violations: Observed employee use personal cell phone then don gloves for food handling, which was corrected on site; observed three-door prep cooler's ambient air tested at 46 degrees with tomatoes at 48 degrees, lettuce at 50 degrees and mayo at 52 degrees, all of which were discarded and education was provided. No non-priority violations.

Jose Locos, 935 N. Glenstone Ave., Springfield. April 26 complaint investigation and routine inspection – Result: active. Three priority violations: Observed vegetable slicer with food debris, which was corrected by washing, rinsing and sanitizing; observed cooked shrimp at 109 degrees, refried beans at 163 degrees and queso at 112 degrees, all of which were cooling on top of kitchen counter without proper cooling method, which was corrected by implementing proper cooling/refrigeration; observed b-wash and hand sanitizer stored on top of cooking equipment and utensils on drying rack, which was corrected by manager moving chemicals to safe location. Three non-priority violations: Observed raw shrimp, raw bacon and raw chorizo stored over ready-to-eat red sauce, pico de gallo and guacamole, which was corrected by manager moving raw meats to safe location; observed cooked shrimp at 109 degrees, queso at 112 degrees and salsa at 49 degrees to 56 degrees on top of kitchen counter, which was corrected by either discarding food or placing it in refrigeration and going forward time control records and documentation will be kept; observed hand-washing sink in kitchen had 84 degree water, bathroom sink water was at 101 degrees.

Karai, 640 W. Republic Road, Springfield. April 29 routine inspection – Result: active. Four priority violations: Observed raw chicken stored above vegetables in kitchen lower prep table and raw fish stored above ready-to-eat sauces in walk-in cooler, which was corrected by manager moving raw meats to safe location; observed two spray bottles filled with bleach solution without labels, which was corrected on site by manager labeling spray bottles; observed three metal tongs and one large metal spoon were dirty on the drying hooks, which was corrected by washing, rinsing and sanitizing items; observed multiple foods in plastic containers and plastic wrap without labels or dates in kitchen prep table and walk-in cooler, which was corrected by labeling and dating food. One non-priority violation: Observed sanitizer strips on site were expired, manager was notified but no other strips were provided during inspection.

Kum & Go No. 1485, 2650 W. Sunshine St., Springfield. April 29 routine inspection – Result: active. No priority violations. One non-priority violation: Observed Rain-X stored over bottled water in storage area, which was corrected by moving Rain-X to a different place.

Leslie's Mexican Supermarket - Restaurant, 216 S. Glenstone Ave., Springfield. April 24 routine inspection – Result: active. Two priority violations: Observed two buckets of salsa prepared over 12 hours prior measured at 54 degrees and 48 degrees inside walk-in cooler, which was corrected by discarding the salsa; observed can opener blade had visible debris buildup, which was corrected on site. No non-priority violations.

Los Michoacanos Garcias, 1402 W. Sunshine St., Springfield. April 24 re-inspection – Result: active. Walk-in temperature now at 38 degrees. No violations noted.

McAlister's Deli, 2445 N. Kansas Expressway, Springfield. April 26 routine inspection – Result: active. One priority violation: Observed quaternary ammonia sanitizer at three-vat sink was 62 degrees and should be 75 degrees or higher. Two non-priority violations: Observed dish sprayer does not shut off and there is a leak under front hand sink; observed kitchen floor is badly pitted and has been a long-term issue that needs to be corrected.

Four priority violations: Observed cheese at 43 degrees, brisket at 43 degrees to 53 degrees, etc., which was corrected by discarding food and repairman had cooler at 39 degrees by end of inspection; observed cheese at 43 degrees and meat at 47 degrees in reach-in cooler, which was corrected by discarding food; observed cheese at 43 degrees and meat at 47 degrees in back reach-in cooler; observed no sanitizer residual in dish machine. The two-door reach-in cooler that was repaired and at proper temperature yesterday was at 67 degrees when inspector arrived; foods inside that were over 41 degrees were discarded, meat thawing in cooler was below 41 degrees and moved to one of the other coolers.
