critical violations found April 9-16 during Richland County restaurant inspections

The following restaurant inspections with critical violations were conducted by Richland Public Health between April 9 and April 16:

Mansfield Christian Elementary School, 500 Logan Road, Mansfield, April 9. Food not properly protected from contamination by separation, packaging and segregation (critical). During inspection, serving station was observed unattended (staff getting chicken patties out of oven and other staff washing hands) as counter full of trays with lunches set out on countertop subject to sneeze, splash, dirt and dust while students wait in line. Sneeze guard should be installed or procedure should be changed by not putting out lunches all at once or having employee staff monitor lunches as they sit on the counter. Correct by May 14. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Tossed salads in high school cooler registered 46 and 49 degrees. Both salads and three additional salads were all voluntarily discarded. TCS foods not being cold held at the proper temperature (critical, corrected). Packaged lettuce in two-door cooler measured at 56 degrees. Person in charge (PIC) voluntarily discarded item.

KV Market, 359 W. Fourth St., Mansfield, April 11. Food package(s) received in poor condition (critical, corrected during inspection). Observed several severely dented cans in the clearance section. PIC removed cans from sale. TCS foods not being cold held at the proper temperature (critical, corrected). Internal temperature of chopped ham and turkey was 63 degrees in the deli case. Internal temperature of chicken and salami in deli case was 52-53 degrees. PIC voluntarily discarded out of temperature lunch meat. Improper storage of poisonous or toxic materials (critical, corrected). Observed baby shampoo on a shelf above baby food in the retail aisles. Missing and/or incomplete allergen declaration(s) (critical, corrected). Observed a package of biscuits in the retail freezer without an ingredients label, common name, net weight and facility responsibility statement. PIC removed biscuits from sale to be labeled.

Maan 2904 LLC (Valero), 750 Lexington Ave., Mansfield, April 11. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed air freshener stored over the ice cream freezer. Ensure all chemicals are stored below or completely separated from foods for sale.

The Richland School Of Academic Arts, 1456 Park Avenue West, Mansfield, April 11. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC could not state cleaning frequency for scissors used for opening of food packages. Ensure scissors are cleaned at a four-hour cleaning frequency. Food contact surfaces not easily cleanable (critical, corrected). Observed scissor that were not separable or easily cleanable. PIC voluntary discarded scissors.

TJ's Hotel, 10 W. Main St., Shiloh, April 11. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed colander in the designated handwashing sink. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed ice for the bar to be uncovered when not in use.

Shell #1357, 1357 Lexington Ave., Mansfield, April 12. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical repeat, corrected during inspection). PIC stated that cappuccino machine cups are washed, rinsed and sanitized then towel dried and left to dry the rest of the way on a paper towel. Ensure that the warewashing process occurs as follows: wash, rinse, sanitize and air dry. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Observed bleach spray in the cabinet to be stored in junction with foods for sale.

Ontario Duchess #1249, 3675 Park Avenue West, Ontario, April 12. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed opened bags of Stok cold brew in self-serve retail dispenser not date marked. Manager was able to verify and properly date marked. Ensure cold brew is date marked to indicate consumption/discard date for six days (seven days total) after opening. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected). Observed insect repellent and dish soap stored among food in clearance box.

Hampton Inn Mansfield/Ontario, 1051 N. Lexington Springmill Road, Mansfield, April 15. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). No signed illness reporting agreement observed for current food employee working. Copy of Richland Public Health's employee illness reporting agreement provided to manager to have employee sign. TCS foods not being cold held at the proper temperature (critical, corrected). Observed multiple TCS foods (cut fruit, whipped topping and pancake batter) not held at 41 degrees or less inside reach-in cooler in kitchen area. Employee voluntarily discarded items. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened gallon of milk held in reach-in cooler in kitchen area not date marked. Employee verified opening and date marked during inspection.

Woodland Elementary, 460 Davis Road, Mansfield, April 15. Food package(s) received in poor condition (critical, corrected during inspection). Can of peaches observed in dry goods room observed with lip dented. Listed item was voluntarily discarded. Food package(s) received in poor condition (critical, corrected). Can of pears stored in dry goods room observed with dents along seam. Listed item was voluntarily discarded by PIC.

Malabar Intermediate School, 205 W. Cook Road, Mansfield, April 16. Unnecessary persons in the operation (critical). Multiple persons observed entering food service facility and no one was limiting access. Proper instruction was given to PIC. Improper cooling of TCS food (critical). PIC stated that she does not keep track of time and temperature during cooling process of leftover food. Proper instruction given to PIC regarding cooling procedure. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Sweet potato puff measured at only 126 degrees on the food line. Listed item was reheated to over 165 degrees before reserving.

Ontario Pointe, 2124 Park Avenue West, Mansfield, April 16. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). Observed 0 ppm chlorine sanitizer during sanitize cycle of mechanical dishwasher. Discussed using dishwasher for wash and rinse only and manually dipping equipment in sanitizer of proper concentration until dishwasher is functioning properly. Correct by April 26. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed an open bag of shredded mozzarella cheese not date marked. PIC discarded cheese.
