Facts: You’ve never seen a Dr. Pimple Popper surgery like this.

In an exclusive teaser for the newest season of Dr. Pimple Popper on TLC, dermatologist and TLC host Dr. Sandra Lee, MD—aka Dr. Pimple Popper—supervises a surgery on Reggie, a man with bulbous neck growths who viewers met in the season premiere episode. But this isn’t any ol’ surgery. In the clip, Reggie is fully awake while a hefty portion of his scalp and neck skin is completely removed.

In the premiere of Dr. Pimple Popper's newest season, Dr. Lee diagnosed Reggie with acne keloidalis nuchae, which is an inflammation of the hair follicle that impacts the back of the head and nape of the neck. According to Healthline, “Acne keloidalis nuchae begins with small, itchy bumps that form around the back of the neck, along the hairline. As time goes on, the tiny bumps become scars, and the hair in and around them falls off. The scars eventually enlarge and look like keloids.” And that’s exactly what happened to Reggie.

Since Reggie’s skin condition occurred over such a large area, Dr. Lee called in a fellow dermatologist with a special skill in suturing to perform the procedure. In the teaser for the Jan. 4 episode of Dr. Pimple Popper, Dr. Lee takes a backseat during Reggie’s procedure, supervising a surgery by Dr. Umar at Dr. U Hair and Skin Clinic. Dr. Lee says she called Dr. Umar because he invented a type of tension suture that will close the area and skirt any possible complications.

Reggie is awake for the procedure, during which a giant flap of skin from the back of his neck. During the surgery, Dr. Umar uses a tool which cuts and cauterizes at the same time, which is essential for scalp surgery.

“The scalp is really notorious for bleeding a lot, so that is what we are starting to see,” Dr. Lee says while narrating the surgery. Thankfully, Dr. Umar’s skill makes removing the massive flap of skin relatively simple with virtually no complications.

“That’s a lot of skin that we removed. You know, that’s a big hole that is left,” Dr. Lee says at the end of the exclusive clip. It certainly is. Yikes.

Watch Reggie’s full procedure—and see how Dr. Umar deals with that “big hole”—on the next episode of Dr. Pimple Popper on TLC, which airs Monday, Jan. 4 at 9 p.m
