In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—pops out an encapsulated pilar cyst from a patient’s scalp. This cyst is pretty oddly shaped, causing some fans to suggest it looks like an autumnal gourd. And you thought fall wasn’t for a few more months…

In the clip, Dr. Lee removes one of the several pilar cysts speckled across the patient’s head. To do so, the famed derm squeezes the growth with her fingers, forcing it out of a tiny incision. The cyst comes out encapsulated, meaning its sac wall is still intact. Dr. Lee simply snips the bottom of growth from the patient’s scalp, allowing the cyst to be removed in one piece.

“Do I get to take it home?” the patient asks.

“She can make a nice necklace out of it,” Dr. Lee replies. “A nice pendant.”

Fans had other ideas, with one commenter calling the growth “a little skin gourd” and several comparing the growth to an eyeball. Dr. Lee even slices into the growth at the end of the video, giving the patient and viewers a look at the creamy frosting-like insides.
