In a new YouTube video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—tackles hundreds of stubborn blackheads forming around a man’s delicate eye area. The blackheads are so densely packed around the patient’s eyes that it earns him a nickname from Dr. Lee: The Masked Man.

The Masked Man has had the collection of blackheads for a couple of years. This is his first treatment with Dr. Lee, leaving the derm to try several techniques for removal. First, she tries simply pushing down with a comedone extractor to no avail.

To help loosen the compacted material, Dr. Lee then pokes the blackheads with a blade to “get them going.” And it works. The blackheads start to wiggle out, chipping away at the “mask” little by little. But some are still stuck, meaning the patient will take Retin-A to help loosen the material and come back for more pops in a few weeks..

Dr. Lee says that often she sees blackheads in a mask-like formation on current or former firemen who get soot trapped in their pores. Though this man wasn’t a fireman, he did work in the air freight business before retirement, often having jet fuel and exhaust blown in his face. Dr. Lee says this likely caused the collection of blackheads.

As Dr. Lee says in the clip, this is the first of several treatments for the patient due to his large collection of blackheads. A blackhead is a single pore filled with dead skin and oil, which forms a black “head” when exposed to air.
