In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—extracts a gigantic blackhead that viewers are calling a “gunkberg.” Similar to an iceberg, there’s more to this gunky growth than first appears on the surface.

In the clip, Dr. Lee first nicks the blackhead to loosen the compacted contents, then uses a comedone extractor to push down on the growth. This forces out a bit of contents, making it easier to grab. Using a little bit of “elbow grease” and some tweezers, Dr. Lee is able to remove the sticky compacted contents in one piece. And it's a big one.

“It’s amazing how tiny it looks on the surface and it’s so huge underneath,” one commenter wrote. “It’s like a little gunkberg.”

A blackhead of this size is commonly called a dilated pore of Winer. A dilated pore of Winer is essentially a large blackhead compacted in a single enlarged pore. This means the large crater left behind by this extraction is actually a single pore that has been stretched from the contents gathering inside. It's truly wild.
