In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—removes a pilar cyst protruding from a man’s head. You’ve likely never seen a scalp cyst like this bulbous egg-like mass.

In the clip, Dr. Lee makes an incision into the man’s head across the length of the cyst. The famed derm is then able to pop the growth out whole, sac intact and everything. This is good news to the patient, as removing the entire sac virtually guarantees the growth is gone for good. With a few snips of her surgical scissors, the encapsulated mass is freed and the patient is ready to be stitched up with precision.

“I love when they come put perfect in little egg shapes,” one commenter wrote in response to the clip.

Pilar cysts are growths on the scalp that contain excess keratin, which is the protein that makes up your hair and nails. Though pilar cysts only have to be removed for cosmetic reasons, it’s obvious this particular specimen likely caused some discomfort for the patient.
