In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—pops a series of blackheads that are connected under the skin in little tunnels. And these are some ooey gooey pops.

The main pop in this video is a stubborn dilated pore of Winer that branches off into tiny soft pops. In the clip, Dr. Lee makes a nick into the dilated pore of Winer to give the compacted contents an avenue of escape. She then pushes down on the large blackhead with a comedone extractor, forcing out its sticky contents.

“They’re sort of borrowed under there,” Dr. Lee tells the patient while struggling with the growth.

After that, Dr. Lee goes to different areas of the skin to pop the other superficial soft pops. Interestingly, Dr. Lee says in the video that these skin concerns are actually all connected under the skin. She adds that the fistulas, or connections between the hollow blackhead spaces, are a result of skin trauma from childhood acne.

A dilated pore of Winer is essentially a large blackhead compacted in a single enlarged pore surrounded by normal skin. This means the large crater left behind by the main extraction in this video is actually a single pore that has been stretched from the contents gathering inside.
