In a new Instagram video, Dr. Pimple Popper—aka dermatologist and TLC host Dr. Sandra Lee, MD—plucks a blackhead from a patient’s lower eyelid. The sensitive removal is sure to have your eyes watering — and Dr. Lee knows it.

“This blackhead is close to the eye, so I know many of you will have to watch through peeled eyes,” Dr. Lee captioned on the clip. “But look how nice it came out!”

In the video, Dr. Lee nicks the blackhead to give the contents an avenue of escape. After that, she uses surgical tweezers to gently squeeze the growth, letting loose a healthy collection of dead skin and oil. It’s strangely satisfying even if it’s a bit gruesome.

Viewers may notice that the patient has significant bruising under the eye, which Dr. Lee explained in the comments of the post.

“The bruising near the eye is due to the fact that the skin under the eye is the thinnest on the body, and as we grow older it gets thinner,” Dr. Lee wrote. “When I numbed the area, it caused a bit a bruising but the bruise went away shortly after.”

A blackhead is a single pore filled with dead skin and oil, which forms a black “head” when exposed to air. The lighter material seen in this clip is actually the same material as the "head," only this skin and oil hasn't been exposed to air.
