In a new Instagram video, Dr. Pimple Popper—aka, dermatologist and TLC host Dr. Sandra Lee, MD—removes a sticky blackhead from a patient’s back, leaving behind a hell of a hole. The compacted growth is actually a dilated pore of Winer, which is essentially a particularly large blackhead. And this one is very, very large indeed.

The clip has no sound, but truly doesn’t need it to be strangely and sickly satisfying. In the video, Dr. Lee nicks the dilated pore of Winer to widen its opening for removal. She then uses a comedone extractor to push out the compacted blackhead, dragging the sticky growth out of its home. What is left behind is a dilated pore that is truly gigantic.

Upon seeing the hole, one fan commented with disbelief, “What do you fill that with? Spackle?”

A dilated pore of Winer essentially a large blackhead, which is defined by a keratin plug in a single enlarged pore surrounded by normal skin. This means the large crater left behind by this extraction is actually a single pore that has been stretched from the compacted contents gathering inside. Someone grab that spackle.

Watch the removal to remember below and see the crater for yourself:
