Portage County Job and Family Services is offering Healthy Aging Grant services to qualified residents as funds remain available, or through June 30.

Services include case management, food assistance, care coordination, transportation assistance and digital assistance.

To be considered for these services, applicants must be at least 60 years of age, a resident of Portage County, and meet the income guidelines of being at or below 300% of the federal poverty level.

The housing portion of the funding for the Healthy Aging Grant is now closed. Any applications that were submitted on or after March 27 asking for assistance with the portion of the program that addresses home repair or other homeowner-based assistance will be denied.

The Healthy Aging Grant Program is statewide and aims to address social determinants of health and foster an improved quality of life for older Ohioans, enabling them to remain in their homes and stay connected to their communities, preserve their personal assets and promote a healthy, independent, active lifestyle.

To apply for funds and/or services through the program, residents are encouraged to obtain an application online at the department's website; in-person at the Portage County Job and Family Services offices on the second floor of the Portage County Administration Building, 449 S. Meridian St. Ravenna; or by phone at 330-389-7512.
