ABOUT RESTAURANT INSPECTIONS
This is a snapshot of the inspections conducted by the Polk County health department.
Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.
Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.
Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

Raw or ready-to eat food is not properly protected from cross contamination, specifically: Bowl of raw chicken is sitting on top of bag of buns and containers of whipping cream in glass reach-in fridge. Point deduction: 5.
Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Several items in glass reach-in by storage room are 50 degrees Fahrenheit (sauce with meat, milk, raw chicken). Operator states that food was prepared about six hours prior. Squirt bottle of cream by stove is 86 degrees. Operator states this has been out for about 30 minutes. Point deduction: 5.

Point deduction: 5.
Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: In glass bar fridge, container of half and half is not date marked, commercial expiration date is Nov. 25, 2023. Container of milk and whipped cream are not date marked. Operator states all three were opened two days prior. No date markings were observed in walk-in fridge. Walk-in is being used to store cooked tempura shrimp and noodles. Operator states these were prepared yesterday. Point deduction: 5.
