ABOUT RESTAURANT INSPECTIONS

This is a snapshot of the inspections conducted by the Marion County health department.

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

Food employees do not use the proper cleaning procedure for washing their hands, specifically: Observed cook line employee swiftly rinse hands under water after handling raw meat and attempt to return to the cook line. Point deduction: 5.

Food-contact surfaces are not cleaned between different raw animal foods, between raw to ready-to-eat food, or as frequently as necessary, specifically: Observed employee wipe cutting board used for raw beef with a dry cloth in preparation to use for sandwich buns. Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Observed the prep cooler opposite the fryers on the cook line improperly holding the following food that an employee stated was stocked the previous night: diced tomatoes (50 degrees Fahrenheit), pulled pork (50 degrees), cooked chicken breast (51 degrees), cooked rice (49 degrees), pico de gallo (51 degrees) and shredded cheese (48 degrees). Point deduction: 5.

Temperature measuring devices are not provided or readily accessible for checking food temperatures, specifically: Probe thermometer is present but malfunctioning and not providing a legible readout. Point deduction: 3.

Holland's Bar & Grill

Location: 2155 Silverton Road NE, Salem

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Observed nine 4-inch deep pans of cooked beans from last night stacked in groups of three uncovered on top of each other on top shelf of walk-in maintaining temperature between 44-75 degrees Fahrenheit. Point deduction: 5.

Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: Observed the following food past seven-day disposition or missing datemarks including: shrimp broth (Dec. 13), cooked beef (Dec. 12), flan, opened milk and Clamato mixer. Point deduction: 5.
