Kourtney Kardashian is currently pregnant with her fourth child (and her first with husband Travis Barker), and the pregnancy hasn’t been without its stressful moments — such as when she was rushed to the hospital for urgent fetal surgery. Kardashian spoke about that moment in a new interview with Vanity Fair Italia, while also opening up about her doctors’ strict rules for her fourth pregnancy, which included no workout and no sex.

“It was terrifying,” Kardashian remembered of the fetal surgery, which required a short hospital stay. “I feel that sooner or later I will go into detail because I hope that it could help others, but I don’t feel ready yet because it was really frightening.” While Kardashian didn’t go into the reason behind the urgent fetal surgery, Mayo Clinic notes that this procedure is done “to help improve the long-term outcome of children with specific birth defects.”

Since the procedure, Kardashian’s outlook on the pregnancy has changed. “Right after the surgery, I reached the point where I let myself go and I stopped worrying,” she said. “Now I talk to the baby every day, have a positive mindset, keep my head straight, and say a lot of prayers. I feel really lucky and grateful.”

And, despite the health scare, Kardashian says she feels “great” physically. At 44, she says this pregnancy is different than her three previous ones in a few ways. For one thing, she’s being looked after by a “group of doctors,” who set some restrictions in place for the first few months. “No workouts, no Pilates, no caffeine and no plane trips,” Kardashian listed. “Even no sex!”

Doctors typically say it’s fine to have sex and exercise during pregnancy, but there are exceptions to the rule. According to Mayo Clinic, sexual activity won’t affect your baby, as long as you don’t have “complications such as preterm labor or placenta problems.” A doctor might recommend avoiding sex if a pregnant person is showing symptoms like unexplained vaginal bleeding, leaking amniotic fluid, premature opening of the cervix, or if there’s a history of preterm labor or premature birth.

It’s also typically safe to work out during pregnancy, “if you are healthy and your pregnancy is normal,” according to the American College of Obstetricians and Gynecologists. However, certain conditions may make it unsafe to exercise during pregnancy, such as some kinds of heart and lung disease, preterm labor, preeclampsia, severe anemia, or being pregnant with twins or triplets. And regardless of pre-existing conditions, ACOG recommends talking to an ob-gyn early in the pregnancy to discuss what forms of exercise are safe to do.

While it’s unclear why Kardashian’s doctors set her rules in place, she said that they ended up heightening her fear. “I think all this caution made me a little afraid because, in the past, I never had to be so careful,” she told Vanity Fair Italia. Recently, though, she says she’s been able to let go of those anxious thoughts. One thing that might help? Her kids’ reaction to the new baby on the way. “They’ve been asking me for another baby for a while,” she said. “When they go to school in the morning, they touch my belly, kiss it, and say goodbye.”
