1 There is a 30-day waiting period, which begins on the date of your enrollment, before you may file a claim. A service fee of $49 may apply. Cracked screen repair protection applies to front screens only, and repair options will be presented at the time of claim approval. Not all options may be available.

2 If your eligible phone experiences a covered breakdown, we'll repair or replace it for you. If we determine that we cannot service your phone as specified in this Plan, we may replace it with a replacement product, or we may at our discretion, issue you a gift card or check based on the replacement value, age and condition of the product, as determined by us, immediately prior to the breakdown.

3 Four claims every rolling 12-months will be permitted for cracked screen repair and extended warranty. Customer contract is considered fulfilled after 4 total claims within a rolling 12-months period and the contract will be cancelled at that time.

Residents of Illinois and Puerto Rico are ineligible for this product
