ELLWOOD CITY — Patients from the former Ellwood City Medical Center have until April 8 to request their medical records before they are destroyed.

A final notice was sent out to patients and parties of the former hospital as part of the ongoing bankruptcy case against Americore Holdings LLC, the former owner of the center.

Americore filed for bankruptcy on Dec. 31, 2019, and the case is still ongoing in the Eastern District of Kentucky U.S. Bankruptcy Court.

Patients and interested parties are asked to reach out to Jeannie O'Donnell, an assistant to the Chapter 11 trustee, at either 972-794-1072, or jodonnell@brileyfin.com.

An "Authorization for Release of Health Information" can either be completed via email or over the phone and signed at medical record pickup.

All records that aren't requested by April 8 will be destroyed.
