Mar. 4—Lemongrass Thai Cuisine, 3830 S. US Hwy 41 (4 Critical, 3 Non-Critical) Observed open employee food found on cooler in dry storage area. Observed raw chicken stored above raw fish and shrimp. Observed several food items without dates of consumption. Observed some food items held past 7 days. Observed table mounted can opener with accumulated debris.

Kroger J-987 & Fuel Center, 4714 S. US Hwy 41 (3 Critical, 5 Non-Critical) Observed raw beef roast stored next to potatoes in same cooler. Found sanitizer at 3-bay sinks in deli/bakery area too low to properly disinfect. Observed hand wash sink in meat area completely blocked by boxes.

Buck's Marathon, 4495 Erie Canal Rd. (2 Critical, 0 Non-Critical) Observed several pre-packaged chicken wraps held past date of consumption. Observed beverage can in hand wash sink.

Asian Grill & Sushi, 3540 S. US Hwy 41 (1 Critical, 4 Non-Critical) Found several ready to eat items without date markings.

Subway, Buck's Marathon, 4491 Erie Canal Rd. (1 Critical, 2 Non-Critical) Observed accumulated debris on soda nozzles in dining room and drive-thru.

Vigo County Jail — Canteen Services, 600 W. Honey Creek Dr. (1 Critical, 2 Non-Critical) Hot water in ware washing machine not reaching proper temperature of 160F to properly sanitize.

Vikings Foodmart, 1160 US Hwy 40 W. (1 Critical, 2 Non-Critical) Observed hand wash sink with accumulated rust, lime scale and other debris. Hand wash sinks shall remain debris free at all times.

Burger King #131, 2575 St. Rd. 46 (1 Critical, 1 Non-Critical) Observed accumulated debris inside ice machine and on soda nozzle in dining area.

University Mart, LLC, 1131 N. 3rd St. (1 Critical, 1 Non-Critical) Observed grab-and-go sandwiches without date markings.
