State troopers are still working to determine what caused one vehicle to veer left of center on an Elmore City area highway resulting in a collision and one death.

The tragic two-vehicle accident came a few minutes after 10 p.m. Monday, May 20 at a site more than a mile east of Elmore City on state Highway 29.

Afterwards it was 40-year-old Lacey Tyler of Elmore City who was pronounced dead when being airlifted from the scene and on the way to an Oklahoma City hospital.

A passenger in Tyler's vehicle, a 12-year-old girl, was taken to a hospital, while the driver of the other vehicle, Jiao Huang, 57, of Elmore City was also injured.

An alert from the Oklahoma Highway Patrol shows Huang was alone while driving a 2008 Toyota Camry eastbound on SH 29.

Coming from the other direction on the same highway were Tyler and the young girl in a 2011 Honda Odyssey.

Troopers report for an "unknown reason" Huang's vehicle went over the highway's center line striking the second vehicle head-on.

Huang was pinned in her vehicle for a reported 48 minutes, while Tyler was in her vehicle for about an hour as both were freed by multiple fire departments using extrication tools.

Huang was airlifted to OU Medical Center in Oklahoma City, where she was admitted in critical condition with head and trunk internal injuries.

An Elmore City EMS ambulance transported the 12-year-old girl to OU Medical Children's Hospital. She was admitted in stable condition with leg injuries.

Troopers did report all three individuals were wearing seat belts when the crash happened.

Assisting troopers were firefighters from the Elmore City, Rush Creek and Wynnewood departments.
