Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Oct. 17 and 19:

Nickel And Bean Coffee Shop LP, 40 E. Main St., Lexington, Oct. 18. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed no signed employee illness agreements on site. Person in charge (PIC) had signed at time of inspection.

Subway #53308, 976 Ohio 97, Bellville, Oct. 18. PIC unable to demonstrate knowledge of relationship of food safety and safe and adequate equipment (critical, corrected during inspection). PIC could not stated how to calibrate food thermometers. Education provided. PIC unable to demonstrate knowledge by having no critical violations (critical). TCS foods not being hot held at the proper temperature (critical, corrected). Observed meatballs being held on the from line at 120 degrees for an undetermined amount of time. PIC voluntarily discarded 2½ pounds of meatballs and sauce. TCS foods not being cold held at the proper temperature (critical, corrected). Observed the following items out of proper cold holding temperature parameters: 5 pounds sliced tomatoes, 2 pounds capicola and 1 pound shredded lettuce. PIC voluntarily discarded all at time of inspection. Presence of live insects (critical). Observed small black flying insects in the following locations: front line wall near BOH entrance door, under prep and three-compartment sink, and under the soda fountain cabinet. PIC stated the facility is serviced by pest control quarterly. Remove all evidence of pest, contact licensed pest control operator and retain records by re-inspection Oct. 25.

The Waterford At Mansfield, 1296 S. Trimble Road, Mansfield, Oct. 18. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Date mark on bag of turkey in the top of the make table cooler was illegible. PIC voluntarily discarded turkey.

Taco Bell #15366, 996 Ashland Road, Mansfield, Oct. 18. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected during inspection). Observed no quat sanitizer located in holding rack.

Hickory Vines LLC, 1745 Hickory Lane, Mansfield, Oct. 19. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical). Observed the sanitizer solution from the dispenser at 100 ppm. Sanitizer to manually adjust concentration not available on site. Correct before follow-up.

Buckeye Community School, 1404 Park Avenue West, Mansfield, Oct. 19. Unnecessary persons in the operation (critical, corrected during inspection). Two students observed in the food prep kitchen. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Ready-to-eat apples observed on serving table unprotected and subject to sneeze, splash, dirt and dust. Listed items removed from counter.

Hedges Campus, 176 Hedges St., Mansfield, Oct. 19. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Two-door cooler ambient temperature measured 55 degrees. Cold-cut sandwiches and single portion dairy milk relocated next to other cooler which was measuring 35 degrees.

Charleys Philly Steaks, 2120 W. Fourth St., Ontario, Oct. 19. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical). PIC states ice cream machine is broken down and cleaned once per week. Discussed that equipment that is in contact with TCS (time/temperature controlled for safety food) and held at proper temperature would be required to be washed, rinsed and sanitized every 24 hours. Also discussed that the dispenser would need to be washed, rinsed and sanitized every four hours. Implement proper cleaning and sanitize frequency for ice cream machine. Correct by Oct. 26. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed metal ladles stored in clean equipment storage area to have water and residue on food contact portion of utensil. Also observed blade on manual can opened to contain visible residue. Facility states ladles are not used anymore. Discussed removing from clean equipment/utensil storage and placing in long term storage or getting rid of unused items. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed TCS (time/temperature controlled for safety) foods (fruit purees and ranch dressing) held above 41 degrees for an undetermined amount of time. PIC discarded listed foods.

Whit's Frozen Custard, 1531 Lexington Ave., Mansfield, Oct. 19. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed a knife with visible residue to be stored in a cup with clean utensils. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed containers of Coney sauce and chicken in the sandwich cooler to both be date marked with a discard date of Oct. 17. PIC discarded at time of inspection. Presence of live insects (critical). Observed small flying insects surrounding the melting wells and the mop sink. Completely remove all evidence and/or contact a licensed pest control operator and retain the visit summary. Correct by re-inspection Oct. 26.

Childtime Childcare Inc., 145 N Trimble Road, Mansfield, Oct. 19. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). Observed bowls of recently prepared barbecue chicken at 111-127 degrees on the cart. PIC placed chicken in the over to reheat to 165 degrees.

Burger King Restaurant #388, 1291 Park Avenue West, Mansfield, Oct. 19. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed scissors used to open food packages that do not come apart easily. PIC removed scissors from service. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed wash water in the three-compartment sink at 103 degrees.
