Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between April 24 and April 30:

Kroger Company #N836, 1500 Lexington Ave., Mansfield, April 24. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical). At time of inspection, PIC states cleaning (wash, rinse, sanitize) frequency of outer surfaces of steam wand on espresso machine in Starbucks area is at the end of every day. Discussed that the outer surfaces of the steam wand are in contact with TCS (time/temperature controlled for safety) foods and stored at room temperature, which would require outer surfaces to be fully cleaned (washed, rinsed and sanitized) at a minimum of every four hours that it is in use. Correct by April 25. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by re-inspection May 2. Handwashing sink not accessible (critical, corrected during inspection).

Observed handwashing sink to be blocked in the produce room by produce carts. TCS foods not being hot held at the proper temperature (critical, corrected). Observed one pan of mashed potatoes in the deli hot-holding display case to have an internal temperature of 123 degrees. PIC was able to verify the product came from the oven less than 30 minutes from observation and reheated the mashed potatoes to over 165 degrees. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed whipped topping stored on counter surface in insulated dispenser in Starbucks to have internal temperature of 55 degrees. PIC voluntarily discarded two bottles/dispensers of whipped topping. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected).

Observed two bags of deli meat to be date marked May 4 and May 8, rendering the marks passed the seven days date mark requirements. Food thermometers are not accurate (critical, corrected). Observed yellow thermometer in the deli being used to temp foods when retrieved from the oven to read 146 degrees while EHSIT thermometer read 133 degrees, rendering the thermometer uncalibrated. PIC took the thermometer out of use and got a new thermometer. Presence of live insects (critical). Observed small- and medium-sized black flying insects in the deli area around the second warewashing sink (one closest to upright coolers). PIC stated the pest control company comes out bimonthly for inspection/treatment. Contact a licensed pest control operator, have invoice/summary for EHSIT review and thoroughly clean area by re-inspection May 2. Food contact surfaces not easily cleanable (critical, corrected).

Observed fraying/peeling on belt of rice press in sushi area, rendering surface no longer easily cleanable. PIC removed belt and discontinued use of machine. Food contact surfaces not easily cleanable (critical, corrected). Observed three pairs of scissors being used in the deli area that are not easily separable for ease of cleaning. PIC removed all scissors from service. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment or utensils are placed (critical). Observed insufficient air gap under the seafood prep sink. Correct by re-inspection May 2.

Gionino's Pizzeria, 1289 Ashland Road, Mansfield, April 24. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener blade with ketchup residue on blade.

Taco Bell #15366, 996 Ashland Road, Mansfield, April 25. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical). Manual wash solution not being maintained at 110 degrees or above (critical). Wash solution temperature was observed no higher than 97 degrees. Correct by April 29.

Ontario Burgers LLC (Culver's), 1364 N. Lexington Springmill Road, Ontario, April 26. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed visible dried residue on food contact surface of scoop stored in clean utensil storage area. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed prepackaged individual portions of cole slaw stored in reach-in counter cooler in front area to be marked for discard April 25. PIC discarded during inspection.

7-Eleven Store # 36178 A, 2424 Possum Run Road, Mansfield, April 26. Handwashing sink not accessible (critical, corrected during inspection). Observed beer cans stored in hands sink basin.

Lexington Central Elementary, 124 Frederick St., Lexington, April 26. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC could not state correct cooling and reheating parameters. Education was provided. Food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility (critical, corrected). Observed employee attempting to wash hands in the prep sink. Ensure all employees understand that handwashing may only occur in the handwashing dedicated sink.

Marco's Pizza #1085, 1305 Lexington Ave., Mansfield, April 29. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer in the three-compartment sink below 200 ppm. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed the top layer of salami over the container fill line in the pizza make table cooler with an internal temperature of 51 degrees. PIC voluntarily discarded the top layer of salami in the container. Ready-to-eat, TCS food not properly discarded when required (critical, corrected).

Observed a container of sausage in the pizza cooler with a use by date of April 24. PIC voluntarily discarded sausage. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed a container of chicken wings in the walk-in cooler without a date mark. PIC voluntarily discarded chicken wings. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed the wash water in the three-compartment sink at 92 degrees.

Starbucks Coffee Co. #2793, 2172 Walker Lake Road, Mansfield, April 30. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical repeat). At time of inspection, employee states outer surfaces of steam wands on espresso machines are wiped down with sani cloth in between uses but only fully washed, rinsed and sanitized at end of day. Employee also states knife and tongs at sandwich/food prep station are switched out every four hours but the metal holder is not. Discussed ensuring any equipment or utensils that are in contact with TCS (time/temperature controlled for safety) foods and stored at room temperature in between use are fully washed, rinsed and sanitized at least every four hours. Correct immediately.

TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed whipped topping inside multiple insulated dispensers sitting on counter at room temperature to be 61 and 68 degrees. PIC discarded whipped topping. Observed gallon of milk sitting on counter to have an internal temperature of 44 degrees. PIC states milk was only left on counter for a short time and placed back in reach-in cooler to cool back down to 41 degrees or less. No air gap or approved backflow prevention device on the plumbing system (critical). No air gap observed at the floor level rim of the floor drain where the drains of the ice maker/storage bin discharge. Correct by May 7.
