Rodent droppings on the bottom of the green display racks located in retail lobby. Instructed person in charge to remove droppings as well as clean and sanitize all affected areas. Instructed person in charge to contact pest control operator. Minimal rodent droppings in office area. Significant rodent droppings and live cockroaches in maintenance closet. Observed significant rodent droppings behind and under standing retail display case. Shelving in pastry cases had chipped paint. Trays within pastry cases cracked. Infestation of rodent throughout facility. Advised person in charge to send proof of corrective action measures. Person in charge failed to ensure that employees follow necessary procedures. Unable to get signature due to staff member becoming aggressive and using profanity toward inspectors.

Result: immediate closure
