Portage County inspections: One restaurant has 18 violations and reduces it to 2 on reinspection

The following are the latest results of Portage County Health Department inspections on licensed food establishments and other types of businesses, which the Stevens Point Journal reports monthly.

Almond

  • Garlow's Bar & Grill, 126 Main St.: March 1: No violations.

  • Raisin Cain, 9606 Akron Ave.: March 14: No violations.

Amherst

  • O'Brien's On Main, 128 Main St.: March 5: Three violations: Establishment has no procedure that addresses responding to vomiting or diarrheal events (corrected during inspection); soap not available at bar handwashing sink (corrected during inspection); bottom of keg cooler has standing liquid and wet soiled rags sitting on the bottom.

  • Smokey Spur, 4180 Smokey: March 18: Four violations: Hermetically sealed pickles and pickled asparagus were in the walk-in cooler and were made by the facility and from an unapproved or unlicensed source (corrected during inspection); sanitizer in the dish machine not being used according to label instructions (corrected during inspection); two bottles of grill cleaner were stored on the prep table next to bottles of oils and spices (corrected during inspection); flooring in front of handwashing sink and dish machine in the kitchen as well as in front of the two-door cooler missing tiles.

Bancroft

  • Pop-A-Top Bar & Grill, 5474 Portage County W: March 5: Five violations: Chili held in cooler exceeded its date mark (corrected during inspection); bleach with lavender scent not approved for food establishment; consumer advisory is missing disclosure statement; slow cooker with low, medium and high setting not approved by the department; areas of kitchen floor have spaces between boards and no longer smooth and easily cleanable.

Plover

  • McDonald's Restaurant, 1205 Commons Circle: March 4: Two violations: Sanitizer near cash registers not used according to label instructions (corrected during inspection); employee working in food preparation area without a hair restraint.

  • O'so Brewing Company Taphouse, 1800 Plover Road: March 29: No violations.

  • Panda Express No. 2392, 260 Crossroads Drive: March 13: No violations.

  • Panera Bread No. 1981, 1250 Commerce Place: March 19: One violation: Sanitizer above three-compartment sink not used according to label instructions.

  • Paradise Bar & Grill, 3401 Post Road: March 18: Two violations: Employee medicine on shelf directly above food prep areas (corrected during inspection); establishment does not have procedures that address responding to vomiting or diarrheal events (corrected during inspection).

  • Super 8, 5246 Harding Ave.: March 1: Three violations: The base frame of sinks in the kitchen constructed of wood that has not been covered, sealed or painted; refrigerator in kitchen is not approved; floor and wall juncture next to door frame in kitchen not coved.

Rosholt

  • Dollar Bills, 158 N. Main St.: March 15: One violation: No procedure to address responding to vomiting or diarrheal events (corrected during inspection).

Stevens Point

  • Arbuckles Eatery & Pub, 1320 Strongs Ave.: Feb. 21: One violation: interior surfaces of ice machine dirty.

  • Arch Angel Cafe at Aspirus Stevens Point Hospital, 900 Illinois Ave.: March 12: No violations.

  • Buffalo Wild Wings & Bar No. 275, 5370 U.S. 10 E. Suite H: March 19: Six violations: Shredded lettuce and sliced tomatoes exceeded discard date; paper towels not available at employee handwashing sink in bar (corrected during inspection); ice scoop stored in standing water; push block of vegetable cutter damaged; plastic soda cups wet stacked together without being dried; several floor tiles missing or cracked in kitchen near fryers.

  • Butter's Brickhause Tavern, 1343 Second St.: March 4: Two violations: Sanitizer not used according to label directions (corrected during inspection); cardboard box of single-use items stored on the floor.

  • B&B's Parkway Bar, 1038 Portage County C: March 13: No violations.

  • CPS Cafeteria, College of Professional Studies Building at the University of Wisconsin-Stevens Point, 1901 Fourth Ave.: Feb. 28: One violation: Croutons and lemon wedges held at customer self-service line not protected from consumer contamination (corrected during inspection).

  • Franny's Family Restaurant & Pancake House, 5720 Windy Drive, Suite D: March 5: Three violations: Whipped butter packets stored at room temperature; ice cream scoops stored in water container between uses (corrected during inspection); clean equipment stored on shelving unit lined with cardboard boxes (corrected during inspection).

  • Jimmy John's, 108 N. Division St., Suite A: March 19: Two violations: Clean food pans being stored without being air dried; employee handwashing sink in kitchen leaking and not in good repair.

  • McDonald's Restaurant, 127 N. Division St.: Feb. 29: Two violations: Soap not available at employee handwashing sink in dishwashing area; wiping clothes laundered on premises without the use of a mechanical dryer.

  • McDonald's Restaurant, 5625 U.S. 10 E.: March 5: Three violations: Cheese slices not held at proper temperature (corrected during inspection); thermometers missing from refrigeration units; reach-in prep line cooler not at proper temperature.

  • Partner's Pub, 2600 Stanley St.: March 6: Seven violations: Sliced tomatoes, lettuce and cooked onions in the two-door prep cooler held at wrong temperature (corrected during inspection); sanitizer in kitchen dish machine not used according to label instructions (corrected during inspection); sanitizer not being used according to label instructions (corrected during instruction); establishment does not have procedures that address responding to vomiting or diarrheal events; chicken noodle soup and chili in cooler with no date marking; right-hand door of two-door prep cooler loose and not properly adjusted; non-food contact surfaces of heat lamp dirty.

  • Subway 103CS, 3296 Church St.: March 6: Three violations: Deli slicer sanitizer not used according to label instructions (corrected during inspection); test kits for checking sanitizer concentrations have expired; toilet in women's restroom not flushing properly.

  • Taco Bell No. 36270, 433 N. Division St.: March 19: Six violations: Employee didn't wash hands after cleaning men's restroom and returning to kitchen workstation (corrected during inspection); beverage station in dining room dirty; clean food pans wet stacked together; waste receptacle in men's room overflowing; several dining tables and chairs dirty; toilet, urinals and handwashing sink in men's restroom dirty.

  • The Big Garlic, 1157 Main St.: March 28: No violations.

  • The OT, 916 Maria Drive: March 19: One violation: Female bathroom not provided with a covered waste receptacle.

Wisconsin Rapids

  • Italias, Romanos Too, 4231 S. 80th St.: March 8: 18 violations: Mozzarella cheese and pork held at room temperature; bacon, raw pork, beef roast, salsa and sour cream in three-door cooler not at proper temperature; designated dish washer unable to demonstrate knowledge of proper sanitization methods for utensils and food contact surfaces; shredded beef roast in three-door cooler and prep cooler not date marked; no thermometer for monitoring temperature in foods; french fry potato press dirty; handwashing sink used for filling and holding employee beverage cups; no approved food protection manager's certificate posted in establishment; drinking cups without covers in food preparation area; employees working in food preparation and cooking area without hair restraint; bags of onions, potatoes and flour stored on floor in preparation room; three-door cooler not holding correct temperatures for cooling unit; grill top and grilling equipment dirty; interior of canopy hood dirty; cooking utensils on grill and grill dirty; steel bins above prep cooler improperly stored; floor and wall juncture next to pizza oven not coved; floor under grill, ovens and tables dirty. Reinspection: March 15: Two violations: Drinking cups without covers in food preparation area; employees working in food preparation and cooking area without a hair restraint.
