Oak Street Health Center opens in Cedar Grove for older adults on Medicare

A large green ribbon was cut Wednesday morning during the grand opening ceremony of the Shreveport Oak Street Health center in the Cedar Grove neighborhood of Shreveport.

“Oak Street Health’s mission is to rebuild healthcare as it should be, which includes expanding our centers across the U.S. to communities where access to high-quality primary care is lacking,” Medical Director Dr. Sevetri Guillaume said.

The ceremony was an introduction of the new practice to the Shreveport. Oak Street Health specializes in caring for older adults on Medicare.

“We deliver personalized and preventive care through a model that was created to meet the unique needs of older adults who face chronic illnesses,” Guillaume said.

Oak Street aims to improve health outcomes, lower medical costs and deliver a better patient experience with a mix of in-center, in-home and telehealth appointments, as well as a 24/7 patient support line.

In addition, there is a community room where older adults can gather for social connection and participate in various activities, including bingo, arts and crafts, chair yoga and educational courses.

“Our community room events offer older adults the opportunity to try new things and socialize in ways they may not have otherwise,” Guillaume said.
