Marathon County inspections: Two restaurants with double-digit violations and 11 ace reports

The following are the latest results of Marathon County Health Department inspections on licensed food establishments and other types of businesses, which the Wausau Daily Herald reports monthly.

Abbotsford

  • Pizza Hut No. 506006, 801 E. Spruce St.: April 23: Three violations: Sanitizer not used according to label instructions; salad dressings moved from original containers into containers for salad bar not properly date marked; ice machine dirty.

Athens

  • Athens Cafe, 220 Alfred St.: April 2: Two violations: Tartar and cocktail sauces in walk-in cooler without date mark; kitchen steam table not sufficient in capacity to reheat foods to proper temperature prior to hot holding.

  • Landmark Bar, 115 Alfred St.: April 30: Seven violations: Walk-in cooler has 10 containers of thawing fish stored on top of two beer kegs (corrected during inspection); sanitizer not used according to label instructions; person in charge not monitoring proper sanitation techniques (corrected during inspection); dish machine unable to dispense correct amount of sanitizer for the washing of dishes; wire shelves in prep table cooler are rusted; no service sink provided for establishment; both doors on lower portion of prep table cooler unable to stay closed without blocking with heavy objects.

Birnamwood

  • Wildlife Campgrounds, 183303 Schwede Lane: April 24: No violations.

Colby

  • Subway, 1210 N. Division St.: April 16: Five violations: Sanitizer not used according to label instructions; utensils in four-compartment ware-washing operation not exposed to sanitizer for the required amount of time; person in charge not monitoring proper sanitization techniques; handwashing sink is not hands-free; drain of ice machine bin not maintained.

Eland

  • Knights Bar, 216160 State 49: April 24: Three violations: Homemade asparagus spears, sauerkraut and relish in walk-in refrigerator (corrected during inspection); no irreversible registering thermometer for measuring surface temperature in dish machine; windows in bathrooms have been replaced but trim has not been finished and sealed.

Marathon

  • Marathon Sports Center, 1200 S. State 107: April 18: Five violations: Thousand island dressing and tarter sauce have exceeded their date marks (corrected during inspection); hamburger and cheeseburger offered undercooked on kids menu; squeeze bottles of various sauces and dressings not date marked in the prep table cooler; food containers and squeeze bottles not labeled with names of contents; cavity or door seals of microwave dirty.

  • McDonald's, 667 Maratech Ave.: April 4: No violations.

Merrill

  • Subway, 1609 Merrill Ave.: April 30: No violations.

Mosinee

  • Lin's Garden, 117 Main St.: April 24: One violation: Three-compartment sink used for thawing chicken, instead of designated prep sink (corrected during inspection).

  • McDonald's, 553 State 153: April 24: No violations.

Rothschild

  • Coral Reef, 1025 E. Grand Ave.: April 5: No violations.

Schofield

  • Arby's, 927 Grand Ave.: April 12: Six violations: Interior of ice machine has two cracks on opposite sides on each side of opening; surfaces of cleaned utensils and equipment have dried food on them; thawing roast beef logs stored under cleaning brushes on wire rack; walk-in freezer contains ice accumulation on ceiling and cooling unit that thaws and drips on stored produce below; employee dropped pen onto floor and placed it back onto clipboard on the wall without washing and sanitizing it; bins used to store clean utensils and equipment contain significant food debris.

  • Country Inn and Suites, 1520 Metro Drive: April 4: One violation: Plastic cups for juice and syrup not stored in original container or not properly protected.

  • Speakeasy, 5505 Business 51: April 11: Five violations: Test kit not available for checking sanitizer concentrations; interior of ice machine dirty; no hot water at handwashing sink in men's bathroom; no handwashing signs provided at handwashing sinks; women's bathroom floor missing tile.

  • Wasabi Teppanyaki and Sushi Bar, 3703 Schofield Ave.: April 18: 14 violations: Portioned salmon filets set on counter and not returned to a cooler in a timely manner; sanitizer in wiping cloth pails not used according to label instructions; person in charge unable to demonstrate knowledge of allergens and their effect on sensitive individuals; person in charge not monitoring food temperatures during cold holding; person in charge not monitoring proper sanitization techniques; fish improperly thawed in vacuum-sealed package; various sauces in squeeze bottles in sushi bar cooler not date marked; water at sushi handwashing sink not under adequate pressure; spray bottle containing chemical cleaning solution not labeled; sushi ingredients in sushi bar cooler stored in unlabeled containers; pan used to catch dripping condensate from vent in two-door cooler; no thermometer for measuring temperature in bar cooler; no thermometer in salad prep cooler; two-door cooler not in good repair.

Stratford

  • Ivan Borntreger, 122450 Big Rapids Road: April 23: One violation: Smelt obtained from an unapproved source.

Unity

  • One More Cast Grill & Bar, 140 N. Front St.: Follow-up inspection: April 17: One violation: Floor and wall juncture along the side and back walls of kitchen not coved.

Wausau

  • Aspirus Coffee Shop, 333 Pine Ridge Blvd.: April 24: No violations.

  • Aspirus Wausau Hospital, 333 Pine Blvd.: April 24: Three violations: Person in charge unable to demonstrate knowledge regarding reheating foods intending for hot holding; employee wearing bracelets on arm while working with food; facility address missing from labeling information on foods offered for retail sale.

  • BB Jacks, 900 Golf Club Road: April 9: Six violations: Employee transferred garlic bread from oven to plate with bare hands; variety of non-shelf stable foods stored in coolers past seven-day shelf life; various food-contact surfaces dirty, including soup cups stored lip down in metal pan covered in food residue; significant number of food-contact containers found not to be certified; various nonfood contact surfaces found to be dirty; light intensity in walk-in cooler insufficient for effectively reading product labels and recognizing unsanitary conditions.

  • Bunkers, 1001 Golf Club Road: April 4: Two violations: Wooden Wisconsin-style cutting board delaminating and can no longer be used; no test kit for monitoring sanitizer concentrations.

  • Chop-Stix Bourbon BBQ & Sushi, 2111 Stewart Ave.: April 24: 10 violations: Employees were witnessed touching food-contact surfaces of customer food containers without washing hands after taking payments and picking up an item off the floor; metal bowls of raw, thawing shrimp placed on top of stack of 50-pound bags of cornstarch; a number of medicine and dietary supplements found stored on top of boxes of single-use plastic straws (corrected during inspection); raw shrimp thawed in metal bowls in open kitchen (corrected during inspection); date marking missing from a variety of refrigerator items, including shrimp tempura, cut carrot, fried shrimp, spicy tuna, greens and portioned wasabi (corrected during inspection); a number of food-contact surfaces of utensils, wares and equipment were found in insanitary conditions with food debris and reside due to insufficient cleaning (corrected during inspection); about 10 working containers of sauces and condiments in food preparation area and walk-in cooler not labeled with common names of contents; cases of whole cabbage stored on kitchen floor (corrected during inspection); a number of nonfood contact surfaces found unsanitary; numerous plastic and metal pans found stacked without first being fully air dried after washing (corrected during inspection).

  • Cop Shoppe Pub, 701 Washington St.: April 9: No violations.

  • Days Inn, 225520 Rib Mountain Drive: April 23: Three violations: plastic used to reheat sausage gravy that is not designed for microwave usage and has become damaged (corrected during inspection); knives on magnetic knife rack in kitchen dirty; current kitchen handwashing sink is a portable sink and not hard plumbed.

  • Fazoli's No. 1753, 3812 Rib Mountain Road: April 3: Seven violations: Dishwasher not dispensing chemical sanitizer at correct concentration; spray bottles containing cleaners and sanitizers hung by the handle on shelving over clean aprons and food tissue papers; person in charge unable to demonstrate knowledge regarding storage of toxic or poisonous materials in food establishment; person in charge not monitoring proper sanitation techniques in the establishment; various squeeze bottles containing sauces and dressings unlabeled; inside and outside of cooling equipment dirty; air gaps on soda fountain ice bins not conforming to plumbing requirements.

  • Hissho Sushi at Festival Foods No. 2692, 110 S. 17th Ave.: April 9: One violation: Raw sushi above other foods in display case (corrected during inspection).

  • Hmong Eggroll, 932 S. Third Ave.: April 4: Five violations: Ready-to-eat foods held in cooler not date marked; egg rolls in coolers and freezers need to be covered or wrapped; containers not labeled with ingredients; area where papaya salads are made dirty; ceiling area in the back store room needs to be repaired.

  • Lee's Famous Recipe Chicken, 2412 Grand Ave.: April 29: No violations.

  • Limericks Public House, 121 Scott St.: April 9: Four violations: Leek soup in reach-in refrigerator has exceeded date mark (corrected during inspection); ware-washing machine not dispensing sanitizer automatically; no approved food protection manager's certificate posted; bar glasses stored inverted on towels.

  • McDonald's, 225500 Rib Mountain Drive: April 18: Two violations: Employees not washing hands after removing soiled single-use gloves; surfaces under equipment dirty.

  • The Mill Yard, 238532 Marathon County W: April 30: Two violations: Sanitizer not used according to label instructions; person in charge not monitoring proper sanitization techniques.

  • Panda Express No. 2462, 4400 Rib Mountain Drive: April 16: Four violations: Various utensils, wares and equipment found dirty; steel wool and green scrub pads for cleaning dishes stored in pan with standing water and food pieces in bottom; recently cleaned plastic containers not fully air dried prior to stacking; single-use drink cups stored unprotected near cash registers.

  • PK Creations, 300 Third St.: April 30: No violations.

  • Rocky Rococo Pizza & Pasta, 226408 Rib Mountain Drive: April 9: Three violations: Cut bell peppers in prep table not held at proper temperature; person in charge not monitoring food temperatures during cold holding; lids on prep table and high-touch points on door leading to dining room dirty.

  • The Sixth Street Filling Station, 1314 N. Sixth St.: April 10: Two violations: Barbecue sauce, citrus sauce and bang bang sauce have exceeded date mark; flexible hose attached to drain of ice machine bin and inserted into floor drain.

  • Subway Sandwiches & Salads, 1819 W. Stewart Ave.: April 1: Four violations: Person in charge unable to demonstrate knowledge of proper cleaning and sanitization methods of food contact surfaces; person in charge unable to demonstrate knowledge regarding use of toxic materials in food establishment; exterior door in receiving area not tight fitting; light intensity in walk-in cooler not sufficient.

  • Taste of Manila, 3737 N. Sixth ST.: April 15: No violations.

  • Thrive Foodery, 2105 N. Second St.: April 11: Two violations: Plumbing on ice bin at soda fountain and food prep sink incorrectly air-gapped; no irreversible registering thermometer to measure surface temperatures in hot-water mechanical ware-washing machine.

  • Yummy Deli, 1205 Merrill Ave.: April 25: Five violations: Packages of hot items were left out past discard time (corrected during inspection); sanitizer in spray bottle not at proper concentration; several refrigerator items stored without date marking; numerous food-contact utensils, wares and equipment stored under countertop where food is prepared and found to be exposed to food residue; various non-food contact surfaces dirty.

Weston

  • Fairfield Inn & Suites, 7100 Stone Ridge Drive: April 24: No violations.

  • The Jim, 3012 Schofield Ave.: April 11: Two violations: No test kit available for monitoring sanitizer concentration in wiping cloth buckets; no handwashing signs at bar or bathroom handwashing sinks.
