  • High Priority - Raw animal food stored over or with unwashed produce. Observed raw shell eggs stored over unwashed onions in the walk-in cooler. The operator properly stored the onions. **Corrected On-Site**

  • High Priority - Toxic substance/chemical improperly stored. Observed spray sanitizer stored hanging near liquor bottles behind the inside bar. The operator properly stored the spray bottle. **Corrected On-Site**

  • High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shrimp stored over ready-to-eat shredded cheese and heavy cream in the right side cook line make table. Operator removed the shrimp. **Corrected On-Site** **Warning**

  • High Priority - Time/temperature control for safety food cold held at greater than 41F. Observed on the cook line in ice: butter (65F - cold holding); cut tomatoes (65F - cold holding); mozzarella (55F - cold holding.) Observed in the pizza make table: shredded cheese (45F - cold holding); sausage (45F - cold holding). Operator stated the food had been out for approximately two hours. Operator placed more ice on the cook line foods and placed ice on the make table foods. **Corrective Action Taken** **Warning**

  • High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed the vacuum breaker on the wrong end of the mop sink splitter. **Warning**

Emergency order recommended: Facility temporarily closed.

Five total violations, with two high-priority violations:

  • High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 25 rodent droppings on the floor of the storage closet accords from the employee restroom. Approximately 15 feet from the kitchen. Observed approximately 40 rodent droppings on the floor and shelves of the establishment's dry storage/alcohol closet. Observed no rodent droppings on any food items. Approximately five feet from the kitchen. Observed approximately 30 rodent droppings in the back water heater/soda set up area approximately 15 feet from the kitchen. Operator stated that they have called Keller's and they are working on the problem. Operator is currently sweeping and mopping the areas. **Warning**

  • High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed a sanitizer bucket at 300ppm chlorine. Operator emptied the bucket out and remade it to 100ppm. **Corrected On-Site** **Warning**

  • High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dishmachine at 0ppm chlorine. Operator switched to their triple sink and measured at 200ppm quaternary.

  • High Priority - Raw animal food stored over or with unwashed produce. Observed raw fish stored on top of a container of uncooked asparagus in the right side make table. Operator moved the asparagus. **Corrected On-Site**

  • High Priority - Toxic substance/chemical improperly stored. Observed their sanitizer bucket stored on top of their make table. Operator removed the bucket. **Corrected On-Site**

  • High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed a sanitizer bucket at 200ppm chlorine. Operator emptied the bucket. **Corrective Action Taken**

  • High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed in front kitchen area employee used cash register, put gloves on hands and grabbed a hot dog bun and failed to wash hands before putting on clean gloves. Educated person in charge on glove use and hand washing and emailed person in charge a copy of the handout. Person in charge discarded bun, removed soiled gloves, washed hands and put on clean gloves. **Corrected On-Site** **Warning**

  • High Priority - Stop sale issued due to food not being in a wholesome, sound condition. Observed in front kitchen area employee used cash register, put gloves on hands and grabbed a hot dog bun and failed to wash hands before putting on clean gloves. Educated person in charge on glove use and hand washing and emailed person in charge a copy of the handout. Person in charge discarded bun, removed soiled gloves, washed hands and put on clean gloves. **Warning**

  • High Priority - Time/temperature control for safety food cold held at greater than 41F. Observed on front kitchen counter: butter (71F - cold holding.) Educated person in charge on time/temperature control for safety food cold held at greater than 41F. Person in charge stated it had been out for approximately 30 minutes and placed it in the reach-in cooler. **Corrective Action Taken** **Warning**

Five total violations, with one high-priority violation:

  • High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed in walk-in cooler raw ground beef over beef flank steak. Educated person in charge on safe refrigerator storage and emailed person in charge a copy of the handout. Person in charge removed raw ground beef. As a repeat violation. the importance of following proper protocol with this violation was reinforced with the employee. **Admin Complaint**
