Season 5 of the reality show premieres next month. In PEOPLE’s exclusive preview of the upcoming episodes, Tammy is finally going home from spending 14 months at Windsor Lane Rehabilitation Center in Gibsonburg, Ohio.

Tammy’s homecoming begins on an upbeat note, with her proudly sharing “I lost 300 lbs. And I’m ready to conquer the world.”

Weight loss is also becoming a family affair as half-siblings Amanda and Misty share that they’re seeking approval for weight-loss surgery, which Tammy had in 2022 and Amy had in 2020.

“Do we get a family discount?” Amy quips to the camera.

Although Tammy’s return home is first met with laughter and levity, the clip also shows the sisters dealing with turmoil at home as Amy struggles to manage the household and her two young children, Gage Dean, 3, and Glenn Allen, 1, while her marriage to Michael Halterman continues to decline.

Amy tearfully tells her family about her problems, which later leads to an explosive confrontation that results in the police being called. “I didn’t know coming home, the world was going to fall apart,” Tammy says in a confessional.

The season 5 premiere of 1000-Lb. Sisters airs Tuesday, Dec. 12 at 9PM ET/PT.
