Springfield restaurant's permit suspended after inspection finds dead mice, infestation

A Springfield restaurant had its permit suspended due to "risks found at time of inspection," according to a July 19 Springfield-Greene County Health Department inspection report.

During a routine inspection, the SGCHD report noted priority violations including a mouse infestation, live cockroaches in the mop sink, and food being held at unsafe temperatures at Bubba's BBQ, 504 N. West Bypass. Non-priority violations outlined included the presence of five dead and decomposing mice on glue board traps as well as holes in the wall "where pests were observed."

"Once the priority and the designated core violations have been corrected, staff should contact our office for a reinspection to see if permit can be reissued," according to the report.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

4D's Eats LLC, Fair Grove. July 18 routine inspection – Result: active. No violations cited during inspection.

Beef A Roo, 1411 W. Kearney St., Springfield. July 17 routine inspection – Result: permitted with re-inspection. Four priority violations: Observed non-food grade buckets being used to move ice, education given and re-inspection set; observed whipped cream that requires refrigeration at room temperature by soft serve machine, which is repeat violation; observed large presence of flies in facility, re-inspection set; observed sliced ham in walk-in cooler labeled with 7-7-23, education given and product discarded. Two non-priority violations: Observed mops stored in mop sink; observed quat sanitizer test strips were ruined, education given.

Break Time No. 3171, 1545 W. Republic Road, Springfield. July 13 routine inspection – Result: permitted with re-inspection. One priority violation: Observed open case cooler holding potentially hazardous foods such as cookie dough, yogurt, sandwiches, cheese and yogurt drinks at 46 to 56 degrees Fahrenheit, which were voluntarily discarded and denatured. One non-priority violation: Observed there was no thin-probe thermometer for cooks to use.

Bubba's BBQ, 504 N. West Bypass, Springfield. July 19 routine inspection – Result: suspend permit. Facility's food permit was suspended due to risks found at time of inspection. Once priority and designated core violations have been corrected, staff should contact SGCHD office for re-inspection to see if permit can be re-issued. Five priority violations: 1. Observed infestation of mice inside facility and live cockroaches beside mop sink. 2. Observed barbecue beans and burnt ends with documented discard time of 3 p.m. still on steam table at 3:15 p.m. Beans and burnt ends measured 105 and 95 degrees, respectively, and were voluntarily discarded. 3. Observed prep refrigerator was broken and pans of potato salad and coleslaw were sitting on trays of ice and measuring at 58 degrees to 60 degrees. Potato salad and coleslaw was discarded at inspector's request. 4. None of refrigerated, ready-to-eat, potentially hazardous foods in the walk-in refrigerator were labeled with a "use-by" date. 5. Dishes were being sanitized in a solution of 10 parts per million of chlorine in the three-vat sink. Sanitizing solutions should be 50 to 100 ppm chlorine. 10 non-priority violations: 1. Observed wood stacked outside had debris built up and could provide harborage for pests. 2. Observed five dead and decomposing mice on glue board traps, four in water heater room and one in dry storage area. 3. Observed no hand soap at front hand sink, which was promptly corrected. 4. Observed no paper towels at front hand sink, which was corrected by staff. 5. Observed pests in holes in wall beside upright freezer and mop sink. 6. Observed significant buildup on floors in kitchen, prep area, water heater room, walk-in refrigerator, hallway to exit, in front of walk-in refrigerator, and especially under and behind equipment. Two box fans also had heavy debris accumulation on them. 7. Observed heavy buildup on inside of hot-hold cabinet, several shelves inside kitchen and smoking area, on the inside and sides of the oven and deep fryers. 8. Observed outside grease dump tank had over an inch of debris on its top surface and was open, so staff scraped debris and put into dumpster. 9. Observed old broken prep refrigerator in parking lot behind facility. 10. Observed several stacks of foil pans that were stored with the food-contact surface upward.

Buffalo Wild Wings - Patton Springfield OPCO LLC, 900 E. Battlefield Road, Springfield. July 18 routine inspection – Result: active. One priority violation: Observed dicer being stored unclean on shelf when required to be clean to sight and touch, and was cleaned at time of inspection. No non-priority violations.

Bullseye Convenience Store, 8374 W. Farm Road 86, Willard. July 19 re-inspection – Result: active. All violations listed on previous inspection have been corrected.

Chinese Chef, 3029 S. Campbell Ave., Springfield. July 17 routine inspection – Result: active. No violations observed at time of inspection.

City Cafe, 2129 W. Republic Road, Springfield. July 17 routine inspection – Result: permitted with re-inspection. City Cafe, 2129 W. Republic Road, Springfield. July 19 re-inspection – Result: active. Priority item corrected, walk-in cooler now below 41 degrees as required.

Classic Rock Coffee, 1900 W. Sunset St., Springfield. July 19 routine inspection – Result: permitted with re-inspection. Two priority violations: 1. Observed three coolers holding potentially hazardous foods above 41 degrees, two at 43 degrees to 44 degrees, and a third in the newer kitchen area at 42 degrees to 47 degrees. A small pan of tomatoes were voluntarily discarded, and the manager called refrigeration company immediately. Items are being transferred to 36 degree walk-in cooler. 2. One priority violation: Observed potentially hazardous foods not being properly cold held on the make table. Tomatoes were at 52 degrees and lettuce at 34 degrees. Items were relocated and a reinspection will be performed.

Freddy's Frozen Custard & Steakburgers, 615 W. El Camino Alto Drive, Springfield. July 17 re-inspection – Result: active. Priority items corrected: Custard machine now holding mix below 41 degrees as required. Time stamps are also on potentially hazardous items on prep cooler. (New prep cooler is on order.)

Four priority violations: Observed fish above 41 degrees in tray of melted ice with no time control, which was corrected on-site and re-inspection scheduled; observed multiple cracked food containers in use and on storage shelf; observed can-opener blade in central grill station found dirty, which was corrected during inspection; observed four No. 10 cans dented on storage rack, which were voluntarily discarded. IHOP No. 5436, 2647 N. Kansas Expressway, Springfield. July 17 routine inspection – Result: permitted with re-inspection. Number of cleaning issues, so a reinspection will be made on or after July 25. Also keep a close watch on the walk-in freezer; some product was slightly soft. No priority violations. Four non-priority violations: Observed about a half-inch of condensation standing at bottom of front prep refrigerator; observed number of issues with areas not being clean, including walls and floor around pop in a box storage, some counters, return air and ceiling vents, walk-in freezer floor and several other areas of the floor and walls in facility; observed some areas of floor in kitchen and dishwashing area have grout and tile in poor repair, plans need to be made to repair these areas; observed some areas of counters, microwave ovens and other surfaces in kitchen and dish area were not clean.

Jamaican Patty Co., 3439 S. Campbell Ave., Springfield. July 14 routine inspection – Result: active. No priority violations. One non-priority violation: Observed daylight coming through bottom of back door.

Jimmy's Egg, 3837 S. Campbell Ave., Springfield. July 18 routine inspection – Result: permitted with re-inspection. Two priority violations: Observed diced tomatoes in pan on melted ice holding at 58 degrees, remainder of pain was voluntarily discarded; observed employee cracking shell eggs on grill, left station to dish room and removed gloves, then put on new gloves without washing hands in between. Employee removed new gloves and washed hands immediately, education provided and corrected on-site. Two non-priority violations: Observed hot water not at 100 degrees at the hand sink on cook line nearest to dish room, manager has work order in with plumbing company; observed back kitchen door not tight fitting, daylight coming through at side of door and door frame, correct by next inspection.

Kearney Street Cafe, 1324 W. Kearney St., Springfield. July 18 routine inspection – Result: active. One priority violation: Observed four No. 10 cans on storage shelf dented, education given and cans voluntarily discarded. No non-priority violations.

Korean Garden, 414 E. Jackson St., Willard. July 18 re-inspection – Result: permitted with re-inspection. Another re-inspection will be made on or after July 25 to check the original violations as well as implementation of time as a control or holding potentially hazardous foods at proper temperature. Inspector stated they observed at least five live cockroaches in one area of dry storage and saw an area that looks to be harboring them as well. The owner told inspector that the facility was sprayed on July 17 for pests. These areas were shown to the owner. Also the sanitizer in the three-vat sink had about 10 ppm chlorine, which is required to be 50-100 ppm. There were still a number of areas of the kitchen and back storage area that need cleaning.

Kung Fu Taco, 3354 S. National Ave., Springfield. July 13 routine inspection – Result: active. No violations noted.

Los Michoacanos Garcias, 1402 W. Sunshine St., Springfield. July 17 routine inspection – Result: active. Three priority violations: 1. Observed multiple food items not date marked, including fired chicken, refried beans, salsa, cooked beef, rice, ground beef, etc., which was a repeat violation and corrected on site. If date marking violation occurs at next routine inspection, risk control plan may be implemented. 2.
