Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 19 and May 23:

Arby's #343, 1155 Park Avenue West, Mansfield, May 19. Person in charge (PIC) did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical, corrected during inspection). PIC verified that temperatures of chicken and other fried foods are not being taken to ensure products are cooked to the correct temperature. Education provided. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness (critical repeat, corrected during inspection). Observed sanitizer buckets and sinks between 0 and 150 ppm. Have unit fixed by re-inspection May 25. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed five bags of cheese sauce in the hot holding unit at 85 degrees. PIC placed back in oven to properly cook to 165 degrees. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed 3.5 ounces of sliced turkey and two cans of whipped cream at inappropriate temperatures. PIC voluntarily discarded.

Pizza Hut of Lexington Ave., 1360 Lexington Ave., Mansfield, May 19. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed corporate employee illness agreement signed; however, form did not include all 13 reportable illnesses nor the symptoms that are reportable. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). Observed marinara at 126-130 degrees. Ensure temperatures of hot holding items are at least 135 degrees or above. PIC voluntarily discarded eight cups of marinara sauce. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed chemical working containers hanging above single service containers.

Ontario Middle School, 447 Shelby Ontario Road, Mansfield, May 19. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Substitute employee had not reviewed employee agreement. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Tossed salad observed on serve line that measured 55 degrees. Tossed salad relocated to walk-in cooler.

Applebee's Neighborhood Grill & Bar, 1023 N. Lexington Springmill Road, Mansfield, May 19. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer solution in three-compartment sink of bar area and in working sanitizer bucket in bar area to be at 0 ppm. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed multiple TCS foods in top of prep cooler held above 41 degrees (cut lettuce, sliced tomatoes, queso cheese, garlic butter). PIC voluntarily discarded listed foods. Employee states lid was not placed onto unit properly over night and large gap was observed prior to arriving.

Ontario High School, 467 Shelby Ontario Road, Mansfield, May 22. PIC did not ensure employees are proplerly maintaining temperatures of TCS foods during hot and cold storage (critical repeat). Several hot and cold holding units did not register calibrated readings. Please schedule regular calibration of all equipment. Improper reheating of food for hot holding (critical, corrected during inspection). Leftover popcorn chicken was reheated to only 145 degrees. PIC instructed to reheat all foods to over 165 degrees.

Wayne's Country Market, 2440 Lexington Ave, Mansfield, May 22. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected during inspection). Observed three sanitizer buckets at 0 ppm. Also observed three-compartment sink near bakery to be dispersing 0 ppm sanitizer. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed can opener with residue on the blade. Improper cooling of TCS food (critical, corrected during inspection). Observed 12 pounds of shredded chicken that was cooled the night before (over 6 total hours) 48 degrees. PIC voluntarily discarded the chicken. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed multiple TCS products (cheese, bacon, meats, sliced tomato, Miracle Whip and cut lettuce) in sandwich make line to be between 44-46 degrees. PIC relocated all TCS items and cooled to appropriate temperatures. Missing and/or incomplete allergen declaration(s) (critical, corrected during inspection). Observed various house packaged items without proper allergen statements (raspberry walnut cream cheese spread, spinach dip and mixed nut and trail mix packages). PIC pulled from sale until proper labeling can be obtained.

Twin Lakes Café, LLC DBA Twin Lakes Lounge, 2220 Marion Ave Road, Mansfield, May 23. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed knife in clean storage area to contain visible food residue. Also discussed ensuring any scissors used to open food products are able to be fully separated to ensure easy cleaning. No scissors observed to be opening food products during inspection, however, multiple sets observed in kitchen area. Private water system not sampled or tested as required (critical). Unable to observe proof of waster sampling analysis. PIC states water was tested in the fall but unable to locate results. Ensure private water system is sampled and tested annually for presence of total coliforms. Correct by May 30. Presence of live rodents (critical). Observed a few rodent droppings near bait trap on floor below prep sink and inside an empty metal can. Facility works with licensed pest control company and observed multiple traps placed for preventative measures. Thoroughly clean areas and monitor for any new activity. Correct by May 30.
