Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between April 14 and April 18:

Bellville Elementary School, 195 School St., Bellville, April 14. Person in charge (PIC) did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical repeat). During inspection, food temperatures not properly monitored and maintained. Unused or returned food improperly served to another customer (critical, corrected during inspection).

Milk put on share cart by students if not used was being re-served again, according to PIC. Milk was discarded. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Caesar salad on serve line was measured at 56 degrees. Listed item was taken of serve line and stored in cold holding walk-in cooler.

La Mexicana Market, 1329 Lexington Ave., Mansfield, April 17. Person in charge did not ensure employees observed foods as they were received (critical, corrected during inspection). PIC verified that temperatures of TCS items are not being taken upon receipt of items. Education provided at time of inspection.

Pulp Juice and Smoothie Bar, 994 Lexington Springmill Road, Ontario, April 17. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed container of chia seeds on front prep counter open when not in use. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed peeling/chipping paint on food contact portion of manual lime juicer/press. PIC discarded juicer.

Penn Station, 2166 Walker Lake Road, Mansfield, April 17. PIC unable to demonstrate knowledge of food temperatures and times for safe cooking (critical). PIC and food employee unable to verbalize safe cooking temperature for both beef and chicken when discussed during inspection. PIC unable to demonstrate knowledge by having no critical violations (critical). Multiple critical violations observed during inspection. Correct by April 24. Written procedures and plans are not maintained and implemented (critical). Observed written time in lieu of temperature policy for blanched fries, however facility was not following policy by providing time mark for when fries were removed from temperature control at time of inspection. Correct by April 24.

Person in charge did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical). Correct by April 24. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Observed working bucket of chlorine sanitizing solution to be at 0 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed container of kitchen utensils in clean storage area to contain visible residues. Raw animal food not cooked to required time and temperature (critical, corrected during inspection). Observed raw chicken cooked to a maximum final temperature of 153 degrees and beef cooked to a maximum final temperature of 134 degrees. PIC had both meats placed back into conveyor oven to finish cooking to an observed temperature of above 165 degrees for chicken and above 155 degrees for beef prior to serving. Improper use of time as a public health control - 4 hours (critical, corrected during inspection).

Observed blanched fries at fry station stored outside of temperature control and under time in lieu policy to not have time marking to indicate 4 hour hold. PIC verified time fries were pulled from temperature control and placed sticker for time mark on container. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed visibly soiled scissors used to open food at prep counter unable to fully disassemble for easy cleaning. PIC removed scissors from use. Insufficient air gap between the flood rim and the water supply inlet (critical). No airgap observed on fountain beverage machine at floor drain located inside cabinet below machine. Correct by April 24.

Malabar Intermediate School, 205 W. Cook Road, Mansfield, April 17. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Tossed salad on serving line measured at 47 degrees. Listed item replaced with lettuce that was cold holding in refrigerator.

Save-a-Lot, 1270 Lexington Ave., Mansfield, April 18. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed food contact portion of the cover that was stored in the clean area in the meat cutting room to have visible residue.

Maan 2904 LLC, 750 Lexington Ave., Mansfield, April 18. Food package(s) received in poor condition (critical, corrected during inspection). Observed a can of Campbell's tomato soup to have dents rendering the inability to guarantee the quality of the product. PIC removed can from sale. Presence of rodents (critical, corrected during inspection). Observed rodent droppings in the back storage room, by the door and behind middle shelving unit. PIC stated pest control is in the facility once a month. Facility is to continue to monitor for signs of activity and increase frequency if needed. PIC had entire area cleaned.

Ontario Duchess #1249, 3675 Park Avenue West, Ontario, April 18. PIC unable to demonstrate knowledge by having no critical violations (critical). Multiple critical violations observed during inspection. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Observed a copy of Richland Public Health's employee illness reporting agreement, however, PIC unable to find signed copies during inspection. Correct by April 25.

Ready-to-eat, TCS food not properly discarded when required (critical repeat, corrected during inspection). Observed open package of sliced American cheese date marked April 10 and past seven day hold time. PIC discarded cheese. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed open package of Johnsonville brats in walk-in cooler not date marked. PIC discarded during inspection.

St. Peter's Catholic High School, 104 W. First St., Mansfield, April 18. PIC unable to demonstrate knowledge of food temperatures and times for safe cooking (critical, corrected during inspection). Correct temperatures were not given when baked beans were required to be reheated to over 165 degrees. Proper instruction given to explain reheating of food when TCS food is below 135 degrees. TCS foods not being hot held at the proper temperature (critical repeat, corrected during inspection). Baked beans on serve line measured at only 115 degrees.
