Inspector Notes: Observed sani bucket and brillo pad in hand sink upon arrival into main kitchen. Also observed no employees must wash hands signage in hand sink area. Observed ice machine baffle with black build up causes black specs on ice. Observed hood fixture above grill, fans and other ceiling fixtures in main kitchen with dripping residue and hanging dust right above open prep top coolers storing proteins (chicken, steak, etc). Observed water residue on floor in bar prep area. Observed dust/debris build up on fan and fixtures in walk in. Observed sheet of flooring in walk in coming up. Observed white container in walk in with major food debris build up.

Inspector Notes: observed raw shell eggs sitting above RTE jalapenos and next to RTE shredded cheese in the walk-in cooler. Observed multiple chemical bottles (glass cleaner, floor cleaner, polish, etc.) stored next to open packages of napkins in the bar area.

Inspector Notes: Observed handwashing sink without soap. Observed mildew-like substance on ice machine. Observed sanitizer dispenser for 3-compartment sink not functioning. Observed in walk-in cooler mildew-like substance and food debris on all food racks. Observed accumulation on fans in walk-in cooler. Observed faucet to handwashing sink in men's restroom not secured to sink basin.

Inspector Notes: Observed raw beef, raw shrimp and raw chicken to be stored next to each other and next to cooked chicken, shrimp and lo mein. These raw items were also directly above raw shelled eggs which was stored next to raw chicken in the walk in cooler. Observed multiple items in reach in cooler and freezer near fryer (chicken wings, boneless, shrimp, wontons, crab sticks) and walk in cooler ( egg rolls, crab sticks) to not be covered. Observed handles of scoops to be stored within the food product in the dry storage area ( flour, rice, and sugar). Observed racks of walk in cooler to be rusted with white debris. Observed flour to be coating the bottom shelving rack in the dry storage area.

Inspector Notes: Observed raw chicken stored above raw shelled eggs, raw pork and a container of lemonade in the reach-in cooler. Observed TCS food items stored (white rice, pig feet, baked chicken, etc.) in the warming cabinet and fried chicken on the steam line hot holding below 135 F.
