Inspector Notes: Observed hand sink by service door to be used as a dump sink (full of ice and tea) at the start of the inspection. Observed none of the hand sinks to be stocked with paper towels or drying provisions at the start of the inspection. Observed multiple "clean" dishes to have old food debris and black speck on them in the clean dish storage shelving ready for use. Observed boxes of vegetables to be stored on the floor in the walk in cooler. Observed ice cream to be stored on the floor of the walk in freezer. Observed box of potatoes and case of canned tomatoes to be stored on the floor of dry storage. Upon inquiry, PIC stated that they were from the truck delivery on Friday.

Inspector Notes: Observed heavy mildew-like build up on interior of ice machine. Observed corroded metal bowl being used for chicken wing sauce. Observed aluminum being used as shelf liner throughout the kitchen area (atop the oven and on the dish rack). Observed facility had quaternary ammonium test strips but no quat being used at sanitizer. Chlorine was used as a sanitizer. Observed heavy grease build-up and debris on floors and walls. Observed 2 blown light bulbs in the hood vent. Observed missing light bulb shields in the hood vent. Observed fruit flies/drain flies throughout the kitchen and bar area.

Inspector Notes: Observed buildup inside bulk ice machine. Observed multiple food items in prep cooler and reach in cooler exceeding 41F that have been held over night (tuna, cabbage, tomato, cheese, peppers, cucumber, lettuce, onion).

Inspector Notes: Reach in cooler with all pancakes and pancake toppings not holding temperature. All items at 43-46 degrees F (strawberries, pancake mix, butter, blueberries etc.) Items in cooler rotated out at 11:00am. Observed time as a public health control items (shell eggs) with no time labeled. Observed employee prepping food while wearing a watch. Observed employee with hair not effectively restrained. Observed employee belongings (cell phones, chargers, ear buds) stored over prep areas.

Inspector Notes: Observed hand washing sink blocked with three carts of dirty dishes in main kitchen. Observed black accumulation on baffle of ice machine. Observed food debris on "clean" utensils and knives in main kitchen area. Observed chicken noodle soup improperly cooled. Soup was at 58F after being cold held overnight.

Inspector Notes: Upon entering the facility observed all employees who work with food not wash their hands before donning on gloves. bserved multiple TCS food (raw chicken, cooked chicken, raw beef, etc.) cold holding above 41F in the reach in cooler next to the handwashing sink and prep top/reach in cooler next to the grill.

Inspector Notes: Observed half eaten pizza in the back in the kitchen, bag of open chips by the cash register, and open soda can on the prep table next to pizza dough. Observed food in the prep top cooler temperature above 41F. Observed multiple ready to eat TCS food throughout the facility not date labeled.
