Inspector Notes: Observed the Certified Food Safety Manager certificate to not be posted in public view. Observed the prep top/reach in cooler next to the grill to not maintain an ambient temperature of 41F. Observed coleslaw to be temped at 49F. Observed no inspection report displayed in public view. Observed the ice scoop handle to be touching the ice in the drink machine. Observed the handle of a scoop touching the coleslaw in the walk-in cooler. Observed the walls and the door on the inside of the reach in coolers to have a buildup of food product. Observed standing water at the bottom of the reach in cooler next to grill.

Inspector Notes: Observed employee actively drinking in the prep area and storing container on the tabletop in the main kitchen. Observed multiple "clean" metal containers on shelving near ice machine and storage area to be grease to the touch and having old food debris. Observed multiple items in walk in cooler and cold holding well to be above 41F. Refer to temperatures. Observed cooked chicken to be stored in hot holding warmer cabinet at temperatures below 135F. PIC stated that the chicken was just placed in unit at 2 p.m.
PAPA JOHN'S PIZZA, 103 MINUS AVE., GARDEN CITY, 80
Inspector Notes: PIC is unable to recall the illnesses and symptoms that requires restriction and/or exclusion from the establishment. Also, unable to access employee health information. Observed rear hand sink with a water temperature at the most 78 F. Observed string cheeses and ranch sitting on the counter with temperatures higher than 41 F. Advised PIC to locate/create time as a public health control procedures for ranch and cheese or to keep food items in the cooler. Also observed alfredo sauce, ranch, and diced tomatoes on the counter with temperatures higher than 41 F. Advised PIC to place food items in the cooler immediately after prep and to cold-hold food item at 41 F or below. Observed approved variance pizza sauce out on room temperature without the variance papers. Observed prep-top cooler door unable to close. Observed dust build-up on fans of walk-in cooler. Observed slow drain at the prep area hand sink.
