Inspector Notes: Observed multiple food items throughout the facility not covered (pasta, lamb, vegetables, lettuce). Observed multiple cooked TCS items throughout the facility not labelled or date marked ( chicken soup, beef stew, pasta, cooked lamb). When inquired about the dates items were cooked, owner informed that it was the previous Friday. Observed build up with the interior of coolers near the steam table. RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Observed staining on the walls throughout the facility. Observed damaged and soiled ceiling tiles throughout the facility (dry storage and main kitchen).

Inspector Notes: Observed multiple food items stored uncovered in walk in (spinach dip, shrimp, corn, bucket of greens, grit cakes, cut sausage, horse radish & spinach dip). Observed food items in walk (cut mushrooms, slice peppers), on shelf (croutons), and in reach in (horse radish and spinach dip) not date marked. Observed two bins of clean dishes stored with dirty. Observed bin of dirty dishes on floor next to walk in. Observed walls and fans of the walk in cooler with dust and soil debris. Observed food debris on cutting boards. Observed debris on inner and outer surface of reach in/prep top cooler. Observed heavy grease and dust accumulation on hood systems.

Inspector Notes: Employee observed touching ready to eat bagels with bare hands when single-use gloves or utensils are required. PIC removed affected food items. Educated PIC on proper glove usage with RTE foods. Small reach-in cooler at front food service holding temperature at 55 degrees F. Cream chees and butter in fridge both 55 degrees F.
