Inspector Notes: Observed mildew like build up on baffle of ice machine. Observed cups being used as scoops in ice bin. Observed no date marking on various food items (chop cucumber, crab meat, mixed veggies, etc.) in reach in coolers under sushi bar. Observed slimy substance dripping from hood system over grill and fryers. Observed food debris build up on prep tops, on counter under microwave, on shelving and wall in server station area.

Inspector Notes: Observed food debris on prep surface above frozen hamburger cooler. Observed prep/reach in cooler with an ambient temperature of 44F. Sliced tomatoes, sliced cheese, and hot dogs were 49F at the time of inspection. Observed grease build-up and food debris on walls and floors. Observed Observed missing floor tiles. Observed water damaged ceiling tiles. Observed build-up on air vents.

Inspector Notes: Observed no paper towels at the main handwashing sink in main kitchen area. Observed TCS foods items held above 41F in prep top cooler (raw pork chops, raw shrimp, raw scallops, raw chicken, pork sausage patties and raw fish). CA: PIC discarded items mentioned above. Advised PIC to have cooler evaluated for maintaining proper temperatures. Observed pans hash browns and bacon stored under grill next to pans of food debris from grill.

Inspector Notes: Observed a buildup of debris on the can opener. Observed a buildup of food debris in the microwaves in the main kitchen. Observed a mildew like substance on the ice baffle in ice maker. Observed reach in cooler holding temperatures from 50F to 67F in main kitchen. Observed no date mark label on cut up melon in walk in cooler. Observed no date label on the smoked salmon in the walk-in cooler.

Inspector Notes: Observed PIC moving trash can away from prep table with gloves on and went back to start prepping the celery. Observed multiple TCS items in the reach in cooler in the main kitchen to exceed 41F (gyro, chicken, tomatoes, etc. goat cheese).

Inspector Notes: Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit in front food service prep cooler. (lettuce, cheese, pico, spinach, lettuce, and corn salsa x 3). Food items placed in cooler at 9:00PM. Facility to use time as a public health control and discard any remaining food product by 6pm. Observed prep coolers and walk in cooler not to have an internal thermometer. Observed an employee working front end service wearing a watch. Observed employee items in stored next to food product (employee food).
