Inspector Notes: Observed all foods in the walk-in (pizza, chicken wings- raw and cooked, rice, diced tomatoes) above 41 degrees F. Observed items in prep-top cooler dated 10/21 and past the 7 day holding period: chorizo, pepperoni slices, bacon, jalapenos, banana peppers, pineapples (reach-in cooler). Observed no date labels on food items within the prep top cooler: meatballs, mushrooms, bell peppers, red onions, pizza sauce and shredded cheese. Observed the walk-in cooler not maintaining temperature (45F ambient temperature). Also observed missing fan/ vent cover and leaking water onto the floor of the walk-in unit.

All food items were prepped in the morning (11am) and are still in the SAFE zone. Observed single-use styrofoam cup without a handle being used as a scoop in the batter container. Observed a single-use plastic ounce cup stored in the cut tomatoes being used as a scoop.

Observed multiple boxes containing apple juice and fruit in the walk in to have mold growing on the outside of the box. Observed chicken being dethawed in standing water.

Inspector Notes: Observed raw beef next to broccoli, RTE sushi fish, cooked chicken and above raw shrimp. Observed TCS food held using time as a public health control with no time marking (teriyaki chicken, sushi rice, fried rice). CA: PIC discarded food.

Inspector Notes: Observed employee water bottle at hand sink upon my arrival into kitchen. Observed employee water bottle near register. Observed employee water bottle above line in grill area. Observed reach in/prep top cooler out of temp. See temp log below. CA COS: PIC discarded all items placed in cooler 24 hours prior to and moved recently prepped items to another reach in cooler until current one fixed. Observed sausage out of temp in steam well. Observed 3 jugs of butter milk in cooler date marked 10/22/23.

Inspector Notes: Observed facility to have an expired CFSM posted from 2020. No one in the facility has a valid certificate. Previous certified food safety manager is in the process of obtaining a new food safety manager certificate. Observed improper separation of TCS food products ( raw chicken stored above beef and next to cut vegetables). Observed multiple plates and containers throughout the facility to food debris on it while stored and ready for use. Observed grease accumulation and old food debris on can opener. Observed green serving bowl to be used as scoop for rice and stored in rice container. Observed shelving units throughout the facility to have grease build up old food debris settled on the bottom of the shelves in the main kitchen.
