Inspector Notes: Observed hand sink used as a dump sink near fry station. Observed raw bacon stored above cooked, RTE burritos and pancakes in the walk-in cooler. Observed mop stored in mop water in main kitchen area not in active use.

Observed utensil stored in front hand-wash sink. - Observed no soap in women's restroom. - Observed hand wash sink in main kitchen makes loud noise during operation - Hand sink at front of house dripping (un-able to completely turn off water). Observed food in reach-in (liquid eggs and shredded cheese) and freezer (mozzarella sticks, pork chops, and hamburger patties) to be uncovered and subject to contamination. Observed multiple items in reach-in (milk, butter, creamer, orange juice, ranch dressing and lemons, see temperature log for specifics) above 41F. (Ambient temperature at approximately 50 degrees). Observed build-up of debris in walk-in cooler. Also observed debris in reach-in cooler and grill equipment area. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Inspector Notes: Observed multiple personal use cups without lids in food prep areas. Rule: Packaged & Unpackaged Food Separation, Packaging, and Segregation: Food shall be protected from cross contamination by: separating raw animal foods during storage, preparation, holding, and display from: Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, Cooked ready-to-eat food. Observed raw tilapia stored under raw chicken. Observed raw tilapia stored next to pre-cooked Salisbury steak in walk in refrigerator. Observed multiple utensils with handles that were melted, cracked or broken, limiting ability to be cleaned and sanitized. Rule: Food Storage - food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination. Observation: Observed flour and sugar in open bags that were not closed, sealed or protected. Rule: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Observation: Observed build up of oil and debris on sides of grills, fryers. RCA: Implement cleaning schedule for material and equipment and train employees in proper cleaning practices. Observed build up of mildew like substance above drink machine on ceiling in main dining area, on tiles in kitchen, and on back splash behind dish washing areas. Walk in cooler did not have sufficient lighting to observe quality of food or date marking labels on food without utilization of a flash light.
