Inspector Notes: Observed clean, ready to use containers including lids under pizza dough prep table to be covered in pizza flour/cornmeal. Observed oil for fryer (next to fryer) and box of pizza sauce (pizza dough prep station) to be stored on the floor of the kitchen area. Observed facility to have expired chlorine test strips for 3 compartment sink. Observed build up on the interior shelving racks and bottom surfaces of reach in coolers. Observed racks throughout the facility to be heavily soiled. Observed oven racks to be have heavy accumulation of food debris (burnt). Observed walls being the oven and prep top cooler to be heavily soiled and have grease, flour and debris accumulation.

Inspector Notes: Observed multiple employee beverages stored on prep areas. (drinks with and without lid and straw). PIC remove beverages. Educated PIC on proper storage of employee beverages. Drinks must have lid and straw. Drinks must be stored under and away from food product, single service items and clean dishes. Drinks cannot be stored over or on prep areas. Observed cleaner and sanitizer stored on prep surfaces, facing single service items destined to go to the public. Also, bottle of spray cleaner handing on rack which held "to-go" cups. Observed multiple employees wearing jewelry while prepping food. Observed employee prepping food while not wearing a beard-net. Observed multiple chargers and ipods stored on prep surfaces.

Inspector Notes: Observed employee drink in prep area on top of cutting board. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. Observed prepped meatballs held past 7 days in reach-in cooler. A food that requires date-marking shall be discarded if it: Exceeds 7 days, not including the time that the product is frozen. Observed sanitizing solution to be high in concentration. Exceeded ppm allowed as per manufacturer's instructions. Observed employee with no hair covering preparing food. RCA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. Observed food debris in bottom of reach-in cooler and around gaskets of cooler. Observed food debris and mold-like build-up in walk-in cooler.

Inspector Notes: Observed no food safety manager certification posted. When certification was requested PIC could not produce and stated that facility did not have a food safety manager. Observed no handwashing cleanser in the hand sink in the back of the kitchen. Observed multiple ready to eat TCS food throughout the facility not date labeled. Observed inspection report posted behind curtains in the storage room. Observed soiled build up throughout the facility and open air gaps with food debris in them.

Inspector Notes: Observed old food labels stuck on stacked, clean containers throught the ware washing area. Observed items (chicken, tomatoes, and feta cheese) holding above 41F in a reach-in cooler on the line. Food items were prepped today. Items in the prep-top on TPHC. Observed an employee phone stored on prep counter in the back prep station near the warewashing area.

Observed white porcelain bowl to be stored in and used as a scoop for flour. Observed ventilation hood to have a heavy accumulation of grease.

Observed no chemical test strips or temperature indicators to be available in ware-washing area.
