The upcoming fifth season of The Kardashians has been billed as showcasing Kourtney Kardashian and Travis Barker‘s “new baby bliss” following the birth of their first child together, a son named Rocky Thirteen, in November. But it’ll also document the reality star’s scary experience with emergency fetal surgery leading up to the big day, as revealed in a new trailer for the show.

Revealed Wednesday (May 8), the teaser features clips of Kourtney smooching the Blink-182 rocker, seemingly backstage at one of his concerts, and later enjoying what appears to be a Disney-themed baby shower with her husband during her pregnancy. “This is the best blessing in the world,” the Poosh founder says. “But it’s also a high-risk pregnancy.”

Later in the trailer, Kourtney is lying down in a hospital gown, holding what looks like Barker’s tattooed hand. “I had to go in for fetal surgery, which was terrifying,” she explains.

Season five of The Kardashians premieres May 23 on Hulu, marking two years since the family dynasty switched over from its long-running reality show Keeping Up With the Kardashians on E!. The trailer also promises to explore matriarch Kris Jenner’s health struggles, Khloé Kardashian’s newfound independence post-split from Tristan Thompson, Kendall and Kylie Jenner’s time at fashion week and more.

The show won’t be the first time Kourtney has spoken about her emergency operation. She first revealed on Instagram in September that she’d underwent surgery, thanking her “incredible doctors for saving our baby’s life.”

At the time, Barker had dropped everything to return home from his world tour with Blink-182, which postponed three shows so that the drummer could be with Kourtney during the process. “I am eternally grateful to my husband who rushed to my side from tour to be with me in the hospital and take care of me afterwards, my rock,” she added in her post.
