Greene Co. food inspections this week include confused employees, restaurants re-opening

Restaurant employees were unable to identify who was in charge and a manager had to be called in, according to a July 25 Springfield-Greene County Health Department restaurant inspection report. Additionally, another restaurant that had closed due to various violations was approved to re-open after a July 21 inspection.

At Kai After Dark, 306 S. Campbell Ave., employees could not identify who was in charge, put a cell phone on a cutting board, and did not follow wash-rinse-sanitize procedures for cleaning equipment. According to the report, "after using food prep equipment, employee ran equipment under warm water and placed equipment into clean equipment bucket." In addition to employee violations, prepared noodles were not being held at the correct temperature in a cooler that was too warm. The restaurant will need to be re-inspected.

Bubba's BBQ at 504 N. West Bypass, which had its permit suspended on July 19, was "permitted with re-inspection" after a follow-up visit on July 21. According to the food inspection report, the "priority violations were substantially corrected," but a few of the violations "need more attention or (were) not able to be confirmed that they have been corrected without food being prepared at this time." A follow-up inspection is set for on or around Aug. 1.

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

Here are food inspections from the past week:

Alamo Drafthouse Cinema, 4005 S. South Ave., Springfield. July 20 routine inspection – Result: active. No violations noted.

Alli's Family Restaurant, 4525 W. Chestnut Expressway, Springfield. July 24 routine inspection – Result: permitted with re-inspection. This inspection was also a complaint inspection and part of the complaint was valid. A re-inspection will be made on or after July 27 to check priority violations and some of the core violations. Please refer to date of correction for details. Two priority violations: Observed five boxes of raw bacon being stored above onions and lemons in walk-in refrigerator, which was promptly corrected by staff moving bacon to lower shelf; observed front refrigerator running warm and following items were from 47 degrees Fahrenheit to 50 degrees Fahrenheit: coleslaw, cottage cheese and milk, which were voluntarily discarded by owner. Six non-priority violations: Observed pile of broken concrete and old ceramic floor tiles on the parking lot near the back of the building; observed loose and damaged seals on at least two chest freezers; observed inside of ice machine not clean; observed outside of toasters, inside of prep refrigerator, fan in salad refrigerator inside and outside of dish machine, outside of ice machine and some counters were not clean; observed holes in floor near walk-in refrigerator and in corner by back oven; observed floor and wall beside dish machine was heavily soiled.

Andy's Frozen Custard Inc., 1300 E. Battlefield Road, Springfield. July 24 routine inspection – Result: active. No violations noted.

Apple Market Deli, 500 S. Miller Road, Willard. July 20 routine inspection – Result: active. No priority violations. Three non-priority violations: Observed a pest-control device installed in kitchen above food prep table; observed back hand-sink was out of paper towels, which was promptly corrected; observed the floor in walk-in freezer (bakery side) was not clean, which was promptly swept but needs a bit more cleaning.

Barnes & Noble Booksellers, Inc., 3055 S. Glenstone Ave., Springfield. July 24 routine inspection – Result: active. No violations noted.

Beef A Roo, 1411 W. Kearney St., Springfield. July 20 re-inspection – Result: active. All priority and non-priority violations cited during routine inspection on July 17 have been corrected and confirmed during July 20 re-inspection.

Black Lab Coffee Company Downtown, 300 W. McDaniel St., Springfield. July 26 routine inspection – Result: active. No priority violations. One non-priority violation: Observed cleaned dishware placed on visibly stained cloth towels for drying, education was provided and corrected on-site.

Boudin & Old Barns Inc., 358 W. Enterprise Court, Fair Grove. July 20 routine inspection – Result: active. No violations cited during inspection.

Bubba's BBQ, 504 N. West Bypass, Springfield. July 21 re-inspection – Result: permitted with re-inspection. Facility was given permission to reopen, since the priority violations were substantially corrected. However, there are a still a few issues that need more attention or not able to be confirmed that they have been corrected without food being prepared at this time. A visit will be made on or about Aug. 1 to verify continued correction and other issues have been addressed. Inspector attached a list of issues that need to be addressed: 1. Sides of stoves and deep fryer need cleaned better. 2. 5 drums of wood ashes need removed from parking lot. 3. Ashes and wood debris needs removed from parking lot. 4. Continue to take actions to minimize pests and remove dead ones. 5. Inside of back oven and hot hold unit need cleaned better. 6. Floor in walk-in refrigerator needs cleaned. 7. Make sure that policies and procedures for time as a control and cleaning schedules are being implemented and monitored to ensure compliance.

Burger King, 3009 S. Campbell Ave., Springfield. July 27 routine inspection – Result: active. Large walk-in freezer repaired from previous routine inspection. Cleaning has increased around floors and fryer area from last routine inspection. No priority violations. Two non-priority violations: Observed door sweep replaced on back kitchen door, however the sweep is bent and there is daylight coming through in two locations, sweep will need to be repaired; observed small walk-in freezer compressor has condensate leak and build-up dripping onto pan on top shelf, all boxes under compressor moved immediately.

Classic Rock Coffee, 1900 W. Sunset St., Springfield. July 25 re-inspection – Result: permitted with re-inspection. One priority violation: Observed front of house prep cooler holding top items at 43 degrees to 44 degrees, bottom food items were at 42 degrees. Back of house small prep cooler was holding items at 44 degrees on top and 42 degrees on bottom. Manager stated coils were cleaned, however will contact repair company and items moved to alternate cooler. Repeat violation.

Drury Inn & Suites, 2715 N. Glenstone Ave., Springfield. July 25 routine inspection – Result: permitted with re-inspection. Two priority violations: Observed can opener not clean, item was removed from service and washed, rinsed and sanitized, which was corrected on site; observed potentially hazardous foods cooled from the night before not below 41 degrees (meatballs at 47 degrees, mashed potatoes at 49 degrees), which were voluntarily discarded.

Dunkin Donuts, 4020 S. Campbell Ave., Springfield. July 24 routine inspection – Result: active. No violation noted.

Fujisan Sushi Bar (Sam's), 745 W. El Camino Alto Drive, Springfield. July 26 routine inspection – Result: active. No violations cited at time of inspection.

Grand Mart, 1310 W. Grant St., Springfield. July 26 routine inspection – Result: active. One priority violation: Observed a bag used to store and sell ice was handled, filled and set inside ice machine on top of ice, education was provided and contaminated ice voluntarily discarded. One non-priority violation: Observed dust and debris build-up on walk-in cooler vents, education provided.

IHOP No. 5436, 2647 N. Kansas Expressway, Springfield. July 25 re-inspection – Result: active. There were still some issues that need to be taken care of as far as cleaning and maintenance, which will be checked again with next routine inspection. No priority violations. Three non-priority violations: Observed the front prep refrigerator still not has been repaired to prevent build-up of condensation but according to person in charge, this is scheduled for later on July 25; observed some areas of floor in kithcen and dish area that have grout and tiles missing or in poor repair; observed area around some ceiling vents and parts of kitchen floor not clean.

Jimmy John's No. 609, 3536 S. National Ave., Springfield. July 20 routine inspection – Result: active. No violations noted.

Joji Quick Stop, 2631 N. Glenstone Ave., Springfield. July 21 routine inspection – Result: active. No priority violations. One non-priority violation. Observed no sanitizer test kit to check concentration of chlorine sanitizer, education given.

Kad-E-Korner Store and Diner, LLC, 2633 E. Shelby Road, Springfield. July 20 routine inspection – Result: active. No violations cited during inspection.

Observed kitchen staff unable to identify who was in charge when asked. Manager was called and asked to come into restaurant; 2. Observed employee had cell phone on top of cutting board used for food prep. Education provided and corrected on site by employee removing cell phone and washing, rinsing and sanitizing cutting board; 3. Observed noodles in cooler tested at 43 degrees, while ambient cooler air tested at 39 degrees. Person in charge stated noodles were prepared the morning before, so noodles were voluntarily discarded. Corrected on site and education provided. 4. Observed noodles test at 47 degrees, ambient air at 52 degrees. Noodles voluntarily discarded and person in charge removed potentially hazardous foods from cooler. Re-inspection required and education provided. Two non-priority violations: Observed menus did not have consumer advisory listed on it, education provided; observed employee fail to perform three steps of wash, rinse and sanitize in correct order (after using food prep equipment, employee ran equipment under warm water and placed equipment into clean equipment bucket), education provided.

Korean Garden, 414 E. Jackson St., Willard. July 25 routine inspection – Result: active. Nothing on time as a control this time, and three-vat sink was not set up. Inspector didn't see any sign of cockroaches during inspection. No priority violations. One non-priority violation: Observed some areas of the floor, especially under and around equipment, was not clean.

Mudhouse Coffee & Tea, 323 S. South Ave., Springfield. July 26 re-inspection – Result: active. All previous cold-holding violations have been corrected, no re-inspection required.

Pasta Express, 3025 W. Republic Road, Springfield. July 26 routine inspection – Result: active. One priority violation: Observed several pans on clean rack with food debris inside, which were removed and washed immediately and corrected on-site. One non-priority violation: Observed vent hood filters have build-up of debris and are not clean, increase cleaning frequency and correct by next routine inspection.

Sam's Southern Eatery 2, 2259 S. Campbell Ave., Springfield. July 24 compliance check 1 – Result: in compliance. No cold holding violations observed.

Springfield Raceway-Main Concession, 2110 N. Farm Road 123, Springfield. July 22 routine inspection – Result: active. No priority violations. One non-priority violation: Observed side of cabinet that holds deep fryers was not clean.

Two priority violations: Observed utensils and food container lids found chipped, cracked or melted, education given and corrected on-site; observed date labeling of food products in walk-in cooler found missing, education given and corrected during inspection. Three non-priority violations: Observed daylight along top and bottom of front and back exterior doors; observed beer kegs stored in women's restroom, education given and corrected during inspection; observed two ice scoops laying directly in ice without handle in upright position, education given and corrected during inspection.

Wessley Enterprise Tropical Sno, 109 E. Jackson St., Willard. July 25 routine inspection – Result: active. No priority violations. Three non-priority violations: Observed drying rack sitting over the hand sink, which was promptly removed at inspector's request; observed a box of disposable cups sitting on the floor; observed no paper towels at hand sink, which was promptly corrected by placing paper napkins nearby until paper towels are replenished.
