Inspector Notes: Observed a bowl of raw eggs on top of cooked beef in the prep top, and raw chicken next to and above, vegetables, shrimp, and soy sauce. Observed raw chicken thawing in stagnant water.

Inspector Notes: Observed employee put on gloves without first washing hands. Also observed employee turn of sink faucet without using a clean barrier after washing hands. Observed cups used as scoops for individual portioning of tartar sauce and other sauces. The scoops had not been used yet so food was not discarded.

Inspector Notes: Observed bar soda gun containing buildup. Observed bar 3-compartment sink with a sanitizing concentration of 100 ppm. Advised PIC to ensure the sink contains a sanitizing concentration between 200 - 400 ppm quat at all times. Observed queso and potatoes above the oven are holding above 41 F.

Inspector Notes: Observed an employee wearing gloves change tasks without washing hands or changing gloves in between task change. Observed no date marking or name labeling practices throughout the facility.

Inspector Notes: Observed employee drink with no lid and straw on top of the prep cooler. Observed Time/Temperature-controlled for Safety (TCS) food product in the walk in cooler and prep top cooler temperature above 41F.

Inspector Notes: Observed opened employee drinks (Monster energy drink cans) stored on a shelf above food prep counter. Observed, in the walk-in cooler, raw chicken wings stored next to cooked chicken and cooked lo mein noodles. Observed the following food items uncovered in the walk-in cooler: egg rolls, cut onions, cut cabbabge, chicken wings. Observed the following food items uncovered in the reach-in units: cooked beef, chicken sticks, chicken wings, and cooked pork. Observed single service containers being used as scoops and stored in flour and sugar. Observed knives being stored in space between reach-in units.
