Inspector Notes: Observed a box of spoiled cucumber with white mold-like growth on it. Observed ice machine interior to have a pink and black build up on where ice is dispensed.

Inspector Notes: Observed front hand sink not easily accessible due to icy machine blocking most of the pathway. Observed middle hand sink to not have hand soap or a hot water knob. Observed middle hand sink to be blocked by multiple pieces of equipment within sink basin. Observed back hand sink to hold a grill brush in it's basin. In reach-in cooler near grill, observed Canadian bacon stored in same holder as a container of liquid eggs and shelled eggs. Carry container does not provide adequate separation between raw eggs and ready-to-eat (RTE) food items. Observed raw bacon stored next to RTE eggs and over Canadian bacon. In walk-in cooler, observed Liquid eggs stored next to Big Mac sauce which was stored over butter chips and coffee creamer. Observed liquid eggs stored on bottom shelf but next to milk. Observed raw shelled eggs stored next to American cheese slices. Observed raw shelled eggs stored in between RTE sauce and butter sticks. Observed same shelled eggs stored over mocha frappe mix and pickles. Observed spoons for coffee drinks stored on drain board of equipment.  Observed brown paper used as shelf liner on walk-in freezer shelves. Observed employee phone and glasses on prep table where wrapped food items were being combined into bags for customer meals. Observed employee item stored on drink equipment touching dried oat containers. Observed employee jacket stored on top of to-go bags.

Inspector's Notes: Observed the following food items to be above the required cold holding temperature of 41°F in multiple reach in coolers and walk in coolers: Fish (46), Crab (45), Shrimp (47), Oyster (45), Chicken (43), Cut Leafy Greens (45), Pork (43). Notified PIC. PIC discarded all items. Inspector's Note: Observed multiple containers of TCS foods (dairy, cut leaf lettuce) without date marking. Notified PIC. PIC discarded all items.
